Job Overview

As part of an integrated, whole health strategy, Cascadia Health embeds Primary Care services within its behavioral health centered programs and services, primarily delivering care at four clinic sites, with additional services throughout the organization. Cascadia is seeking a Program Medical Director, Primary Care to oversee the delivery of medical care across this continuum of care.

The Program Medical Director, Primary Care, in conjunction with the medical team, will provide Primary Care diagnosis and treatment to clients of all life cycles. The Primary CareMedical Director will provide clinical supervision and leadership to Primary Care medical staff. This position will assist the Chief Medical Officer and Primary Care Director with design, implementation, and continuous improvement of Primary Care services, with a customer service approach to Cascadia's clients and behavioral health services that reflects trauma informed and culturally relevant and specific approaches. The Primary CareMedical Director will further assist the Chief Medical Officer with the scope of service design and quality assurance to overcome the significant health inequities experienced by individuals affected by mental health and substance use disorders.

Under the direction of the Chief Medical Officer (CMO) works in matrixed collaboration with the Patient Care management (PCM), Utilization Management (UM) Management, and the Health Operations teams. The Associate Medical Director is responsible for performing PCM and UM program duties, and other clinical practice improvement initiatives as defined and requested. This position may be assigned to lead medical management and Primary Care population health initiatives for the organization including clinical quality in coordinated care and the Coordinated Care Organization (CCO) / provider network, as well as utilization, quality, case management, and disease management operations, monitoring, and results. The position will provide ground level clinical direction in support of call clinical programs, transformation, and process improvement efforts.

Essential Duties

  • Provide Primary Care diagnosis and treatment integrated within Cascadia's behavioral healthcare services, in a manner consistent with best practices and standards of care.
  • Integrate best practices and/or protocols into the day-to-day clinical and administrative operations, including approaches to culturally relevant and specific services and Trauma Informed Care.
  • Incorporate principles of equity, diversity, and inclusion in support of clients, colleagues, and the community in which we serve.
  • Play a leading role in the recruiting and retention of medical staff, including advising the Chief Medical Officer and Primary Care Director in medical staffing requirements and scheduling medical coverage.
  • Partner with the Primary Care Officer and Chief Medical Officer in the selection and evaluation of provider staff.
  • Provide formal supervision, consultation, and peer review for all of Cascadia's Primary Care providers.
  • Provide Primary Care consultation to Cascadia's psychiatric providers and behavioral health clinicians.
  • Provide technical advice to the Primary Care Director on matters of medical judgment.
  • Advise Cascadia's Integrated Care leadership and staff in the development of wellness programs.
  • Review quality of care provided by Primary Care medical personnel through periodic chart review, and quality improvement dashboards.
  • Determine the need for continuing education opportunities for medical personnel and assist with arrangements.
  • Evaluate specialist referral activity and assist in the development of referral relationships and arrangements.
  • Partner with in-house pharmacy for the evaluation and development of a pharmacy formulary, as well as a preferred labs list.
  • Work with Primary Care Director and senior leadership to define goals and review the progress of integrated Primary Care operations.
  • Demonstrate responsibility for the safety of consumers and/or residents, staff, and property; demonstrate familiarity with fire regulations and evacuation procedures.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Provide input on all relevant documentation and procedural requirements.
  • Deliver timely, accurate, and professional documentation.
  • Complete annual employee training requirements on a timely basis per agency requirements. Attend seminars, training, and other educational opportunities in order to develop professional skills and abilities.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
  • Perform other job-related duties as assigned.

  • Determine staffing requirements, recruit, hire, and train staff in a manner that encourages and maintains diversity and cultural responsiveness.

  • Establish and manage budgets for assigned programs and monitor monthly expenses.

Qualifications

Education:

Must have either of the following:

  • Active state medical license as a Family Medicine or Internal Medicine / Primary Care MD or DO, including maintenance requirements and DEA certification.

Experience:

Five years of post-residency experience required

Experience working in an FQHC is required

Leadership / administrative experience is preferred.

Experience in community health clinics, community mental health setting, or with houseless and special needs populations is highly preferred.

Specialized Knowledge, Skills, and Abilities:

  • Sufficient computer skills to complete documentation in an electronic medical records system in a timely manner
  • Provide care in a setting consistent with the principles of collaborative care informed by stages of change models

Other:

Ability to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current.

Working Conditions

Work is performed in a typical clinical setting. As such, the setting is not secure, and the LMP will be responsible for creating a safe environment within the area. In some cases, safety personnel may be available to assist with this goal. Primary care providers will be provided the appropriate tools with which to meet the requirements of their work (e.g.- computer, printer, scale, medical equipment, and supplies, etc.). Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise, and contaminants.

Mental Demands:

This position must be able to handle a variable workflow, manage time effectively, prioritize activities, and help Primary Care staff with whatever tasks are necessary to establish this new Primary Care capacity. The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach.

Physical Demands:

The Program Medical Director, Primary Care must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations. This position must be able to escort clients between waiting areas and their meeting space, before and after the encounter.
