ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: other duties may also be assigned:
Supervision:
  • Provide direct supervision and daily assignments to program staff.
  • Perform supervisory responsibilities in accordance with organization's policy and applicable laws, as directed.
  • Planning, assigning work and directing/coordinating staff.
  • Ensuring community-based staff are fulfilling duties and providing meaningful services to individuals served.
  • Supervise the documentation of individual participation and case management performance from staff.
  • Implement employee training schedule in coordination with supervisor.
  • Address concerns/complaints and problem solve with supervisor.

Direct Service:
  • Understand and embrace the core concepts of Person-Centered Thinking and the Mission, Vision & Values of the agency.
  • Perform work activities in conformance with Easter Seals values which include Respect, Integrity, Compassion, Stewardship, Learning, Teamwork and Entrepreneurial Spirit.
  • Welcome, value, and embrace cultural diversity in all areas of Easter Seals, i.e. customers, volunteers, employees, individuals served and those that support them.
  • Follow all safety procedures as outlined by the Injury, Illness and Prevention Program Safety Manual and understand the consequences of non-compliance.
  • Provide exceptional customer service both internally and externally.
  • Maintain all needed documentation and records, meeting HIPAA, PHI and other confidentiality requirements as defined by law and/or Easter Seals policies and procedures.
  • 5 hours per week. Variable days and times as necessary to work with individual, staff, agency representatives and other stakeholders.

Job Duties
  • Serve as a member of the Person-Centered Planning team and assist with curriculum development to meet program goals and objectives to support extensive community integration of individuals.
  • Ensure Day Program staff have received Person Centered Thinking (PCT) Training prior to beginning interaction with individuals served.
  • Train Day Program staff to employ PCT skills and help individuals make informed decisions.
  • Provide support to Day Program staff in building PCPs for individuals served as needed.
  • Serve as a back-up Day Program staff when necessary.
  • Participate in Person Centered Plan (PCP) meetings when needed and as time permits.
  • Use Matching tools with Day Program staff and individuals to coordinate successful matches in line with an individual's PCP.
  • Working in conjunction with Day Program staff; tracking when PCP meetings are occurring and following up to ensure plans are implemented.
  • Coordinate the evaluation and documentation of individuals PCT documents and progress towards meeting goals as defined in the individual's ISP.
  • Assure that all required reports and documentation (monthly flow sheets, ISPs, monthly reports, daily documentation, etc.) are completed on schedule and submitted to required parties. Ensure documentation meets Easter Seals standards.
  • Review and update all relevant individual data and maintain files as required by Easter Seals, Alta Regional, and Department of Developmental Services standards.
  • Develop awareness of volunteer and/or employment opportunities in the community and share with interested individuals.
  • Network with community figures/organizations/businesses to build and maintain relationships and foster connections that support diversified community integration opportunities for the people we serve.
  • Market the services the program offers by meeting with potential participants, families, Service Coordinators, advocates, schools, etc.
  • Provide opportunities for individuals to develop skills and increase functional abilities in the areas of domestic living, recreation/leisure, community integration, communication and vocational skills.
  • Ensure that interactions between individuals served and Day Program staff are conducted in environments that are safe.
  • Ensure that community-based services are safe and clean and meets the standards of quality in all programs per Easter Seals quality and compliance standards.
  • Assist in developing annual budget and monitor monthly financial statements in cooperation with supervisor.
  • Utilize technology platforms to ensure efficiency, continuity, and adhere to auditing standards Easter Seals follows.

Meetings:
  • Lead and participate in general staff meetings on a weekly basis.
  • Coordinate annual and semi-annual ISP meetings to review individual's plans as needed.
  • Attend and participate in monthly Program Supervisor's meetings.
  • Attend and participate in meetings/trainings held by Easter Seals' governing organizations or others demonstrating professional growth opportunities, and other meetings as assigned.

Requirements
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education:
  • Minimum of an Associate of Arts degree in a field related to health and human services, special education, welfare, rehabilitation or business, from an accredited college or university is recommended; a Bachelor's degree is desirable.
  • Experience in the supervision or management of a human services delivery system.
  • Two (2) years' experience working with individuals with developmental disabilities.
  • The demonstrated ability to provide staff training, supervision, and planning.

Working Knowledge of:
  • General principles and practices of supervision and staff development.
  • Philosophies, objectives, and goals of community service organizations.
  • Disabilities and related characteristics of individuals served by the agency.
  • Effective interpersonal communication techniques.
  • Community resources, related agencies, and available services.
  • Skills required to ensure organization of workload.
  • Intermediate computer skills using word processing and data management programs.

Language Skills:
  • Communicate effectively verbally and in writing.
  • Follow written and verbal directions.
  • Effectively be able to present information and respond to questions from supervisor, co-workers, individuals, and the general public.

Mathematical Skills:
  • Calculate figures and amounts such as addition, subtraction, multiplication, division, and percentages.
  • Ability to apply basic math concepts.

Reasoning Ability:
  • Ability to solve practical problems and use available resources.
  • Work independently with minimum supervision.
  • Interpret and follow a variety of instructions provided verbally, written, in diagram and/or schedule form.
  • Maintain composure and good judgment under stressful conditions.
  • Establish and maintain effective working relationships with staff, volunteers, individuals served, families, care providers and the general public.

Certificates, Licenses, Registrations:
  • Position requires possession of a current valid California driver's license, proof of liability insurance and driving record that meets the agency standards.
  • Requires a current certificate in Adult First Aid/CPR with AED from an approved provider.
  • Requires current fingerprint clearance through the Department of Justice.
  • Position requires a pre-placement physical.
  • Position requires proof of negative TB test within the twelve (12) months before hiring.
  • Evidence of full COVID-19 vaccination status as documented by Vaccination Record and includes the passing of two (2) weeks from the last required dose.

While performing the duties of this job, the employee is regularly required to:
  • Walk, sit, stand and lift or carry items not weighing more than 25 lbs.
  • Safely push adult clients in wheelchairs consistent with the organization's safety policies.
  • Safely transfer, position and lift adult clients using proper lifting techniques or devices, and lift teams, as appropriate for the specific client and consistent with the organization's safety policies.
  • Safely use wheelchair lifts and perform tie-downs.
  • Talk and hear.
  • Use hands and fingers to handle, feel objects, tools or controls.
  • Occasionally, the employee must reach for items, crouch or kneel.
  • Successful performance requires specific vision abilities that include close to far vision, to include depth perception and visual accuracy.
