NewYork-Presbyterian has a long tradition of bringing hope, health and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities, and is strengthening those that exist. Our programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Program Coordinator to help drive this work forward.
Program Coordinator – HIV/AIDs Program –Weill Cornell ACN
In this key role, you will work with individuals who are living with HIV, ensuring they are linked to medical care, and provide essential care coordination services within an outpatient, multi-disciplinary setting. Services such as benefits navigation, advocacy, referrals to community-based services, treatment and medication adherence counseling, as well as outreach services. Complete documentation and meet program deliverables. Conduct home visits as needed. Bilingual and experience with the population preferred.
Preferred Criteria
- Bilingual Spanish/English
- Master's Degree in a related field
Required Criteria
- Bachelor's Degree or minimum of 3 years of equivalent experience
- Proficiency in Microsoft Word
- Strong interpersonal skills, highly organized, and able to work with diverse community groups and multi-task

Salary Range:
