CHC: Creating Healthier Communities exists so that every person in every community has an opportunity to live their healthiest lives. Together with partners, we’ve been creating healthier communities for more than 65 years.

Since launching in 2021, CHC’s Health Equity portfolio has grown to more than $9 million in impactful, community-level responses. Our partnerships are focused on centering the voices of community members who are most expert and most proximate to health barriers; resourcing those individuals and institutions that can create lasting change in communities that have long been under-resourced. Our partnerships are forged to demonstrate provable, scalable results that eliminate funding cliffs and create the chance to address the root causes of social and structural barriers limiting health and wellness potential.

The Program Assistantwill support and help drive the work of the Equity & Impact Department, whose role is to advance the vision and mission of CHC through program development, participatory grantmaking, data analytics and capacity enhancing resources, that support community-led approaches to address the social drivers of health that are leading to increased health inequities.
