This position is responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost effective care for the identified patient population. Participate in the training and development of the Social Worker Case Manager I.
Minimum Qualifications
Education/Specialized Training/Licensure: M.S.W., L.M.S.W.
Work Experience (Years and Area): 2 years Medical Social Work
