PRN Registered Nurse

Options for Senior America, a 32 year-old home health care agency, is seeking a PRN Registered Nurse for our Wayne office. Job duties and requirements are as follows:

Job duties:

· Conduct client Needs Assessments and develop Care Plans for new care recipients before services start.

· Conduct documented home visits every 90 days.

· Assess the changing needs of care recipients.

· Determine appropriateness of level of care being provided.

· Follow up with client families for whom home visitations were conducted in the event that changes to the care arrangements are deemed necessary.

· Confer with and advise staff on any medical (in nature) incidents/complaints generated by Client.

Job Requirements:

· Must be a Pennsylvania State licensed Registered Nurse

· Must have at least three years of nursing experience (home health preferred)

· Must be dedicated, reliable and sensitive to the needs of the elderly

· Must be willing to travel to and around Delaware, Chester, Philadelphia and Montgomery PA counties

· Driver’s license and personally owned vehicle preferred
