Job Description:
We're looking for a Board-Certified Nurse Practitioner who is passionate about Primary Care and has completed a collaborative agreement in New York State. This role involves working independently and as part of a team, delivering high-quality care and integrating with specialists to provide a comprehensive healthcare experience.
Key Responsibilities:
- Perform patient assessments, diagnose conditions, and develop treatment plans in a Primary Care setting.
- Prescribe medication under a completed collaborative agreement and monitor responses to treatment.
- Educate and advise patients on health management, disease prevention, and lifestyle modifications.
- Work in tandem with healthcare specialists to deliver holistic care.
- Ensure accurate record-keeping through diligent documentation in EMRs.
- Engage in continuous learning to keep abreast of medical advancements.
Qualifications:
- Active Nurse Practitioner license in New York State with Board Certification (FNP or AGPCNP).
- A completed collaborative agreement in New York State, allowing for the prescription of medications.
- At least 2 years of experience in Primary Care, demonstrating autonomy and excellence in patient care.
- Exceptional communication skills and the ability to collaborate with a multidisciplinary team.
- Proficient in the use of EMR systems and technology in healthcare.
