Position Description

Upper Room AIDS Ministry (URAM). URAM provides healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible for performing their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service.

Reporting to the Medical Director, the Nursing Director is responsible for planning, organizing, and directing the day-to-day clinical operations for Primary Care, specialty care (podiatry and Cardiology), and mobile health. This includes scheduling, management and oversight of patient flow. The Nursing Director will work collaboratively with clinical directors of all departments, coordinators and the entire health service management team to ensure compliance with all regulatory/contractual guidelines for Article 28, 31 and FHQC-H.

The Nursing Director is as a team leader and is responsible for managing, coordinating, and educating multiple patient care team members including, but not limited to, Nurses, Medical Office Assistants (MOAs). Duties include, but are not limited to, mentoring, evaluating, and training staff. In addition to the supervisory role, the Director of Clinical Operations oversees, infection control, central sterilization processing, risk management and occupational health for the entire health services unit.

Essential Duties and Responsibilities

Essential duties and responsibilities include, but are not limited to, the following:

Administrative

  • Manages clinical services for all health services sites (The Nest, Willis Green, Mobile Units) to ensure the delivery of culturally competent, evidence-based, patient-centered care by all team members.
  • Oversees clinical operations by providing direct supervision to, and coordination of, Nurses, Medical Office Assistants.
  • Works with the management team to plan, organize, and direct all clinical services within each department.
  • Assists in the development and revision of clinical policies and procedures.
  • Defines the philosophy, purpose, policies and procedures, and standards for RN, LPN, Medical Offices Assistants, and dental assistants in collaboration with department directors.
  • Ensures adequate RN, LPN, and MOA staffing and competency.
  • Generates and implements both an onboarding and a continuing education curriculum for RN’s, LPN’s, MOA’s aimed at maximizing staff retention and professional development.
    1. Fosters an environment in which each RN, LPN, and M)A practices to the top of their license, certification, and/or skillset and role.
    2. Ensure appropriate training, error monitoring and correction are completed in a timely manner.
    3. Conduct staff evaluations on an annual basis
  • Attends all required meetings; these include, but may not be limited to:
    1. Health Services Management Team Meeting
    2. Individual Department Staff Meeting
    3. All-Staff Meeting
    4. Risk Management Meeting
    5. Complex Case Meetings
    6. Quality and Population Health Meetings
  • Introduces innovations by piloting new roles and care delivery models with the primary purpose of making patient care more safe, effective, timely, efficient, equitable, patient-centered, and family-centered.
  • Supervisory duties, including but not limited to:
    1. Reviews and evaluates charts, and any additional records pertaining to client care to ensure accurate, timely and complete documentation.
    2. Ensures assigned staff members complete the following tasks.
  • Hospital discharge follow up calls and updates log.
  • Call4Health database monitoring
  • Maintains Injectables database, ensures patients have scheduled appointments and medications are on site.
  • Appropriate VFC/VFA tracking and documentation.
  • Reporting of communicable diseases to the department of health
  • Maintaining database and tracking patients on Buprenorphine
  • Laboratory reports and manual reconciliation of labs as needed.
  • Timely approves and submits payroll for assigned staff.
  • Coordinates provider and staff schedules, including working with the nursing supervisor, lead MOA and program manager to coordinate staff schedule.
  • Partners with Human Resources Department on all personnel related matters: hiring, orienting/training, retaining and, when appropriate, terminating process. Orient and support all clinic staff.
  • Oversees patient flow through Primary Care, behavioral health, and dental clinics
  • Acts as a liaison between the organization and community partners
  • Acts as a liaison with vendors such as Bioreference and Stericycle
  • Collaborates with other department Integrated Harm Reduction, Business Development, Health Home, Adult Day Health Care Program, Housing

Quality Assurance

  • Revises and maintains the organization’s infection control and occupational health policies.
  • Enforces adequate Infection Control standards, with guidance from the Medical Director
  • Oversees the nursing team responsible for:
    • Ensuring all clinical equipment is working properly.
    • Medications, Vaccines and Clinical Supplies Inventory
    • Monitoring Clinical logs
    • Environmental rounds
    • CLIA WAIVED Testing Logs
  • Oversees the Vaccine for Children, Vaccine for Adults and other vaccine programs.
  • Acts as point of contact and coordinator for the New York Citywide Immunization Registry (CIR)
  • Works in Collaboration with the Senior Director of Special Projects in issues related to patient safety and risk management.
    • Reviews and acts on incident reports and directly implements action plans or ensures responsible managers do.
    • Maintains and monitors incident reports logs.
  • Ensure annual OSHA training is completed and Conducts rounds to ensure compliance with OSHA safety standards.
  • Works in Collaboration with the Quality Improvement and Population Health team
  • Collaborates with other Management Team members as well as other clinical leaders within the organization on issues of patient care, clinical quality, compliance, customer service, utilization management, care management, patient access, system redesign, safety, and health center strategy.
  • Participates in the organization’s readiness programs for regulatory surveys, including but not limited PCHM, FTCA, and HRSA, DOHMH, AIDS Institute site visits.
  • Works in Collaboration with Human Resources, Medical Director and Senior Director of Special Projects in assisting in the implementation/maintenance of NYSDOH regulations such as but not limited to: Influenza and other vaccinations, Influenza Mask Protocol, Fit testing and other policies related to infectious diseases.

Other duties as assigned by supervisor.

*This position is primarily an administrative/leadership role. For Nurse Practitioners there are no expectations to provide direct patient care or hold a patient panel.

Qualifications/Education/Experience

Requirements

  • Master’s degree or higher required. Nurse Practitioner Preferred
  • Current NYS license
  • At least five years of experience in the Primary Care setting
  • At least three years of experience in a management/leadership role
  • Basic Life Support Certification

Special Skills and Knowledge

  • Superior written and verbal communication skills
  • Ability to work as an efficient team member, under pressure and deadlines.
  • Experience with working at Federally Qualified Health Centers or other low resource settings.
  • Knowledge of NYS and federal requirements for Article 28 and 31 health centers.
  • Knowledge of HRSA Requirements for Health Centers
  • Deliver excellent customer service reflecting a commitment to caring, courtesy, and respect for all patients upholding the mission of the organization.
  • Ability to present information in one-on-one and small group situations.
  • Bilingual preferred (English/Spanish or English/French)
  • Experience in caring for people with experiences of homelessness, people who are living with HIV, people who use substances, including people who inject drugs.
  • Experience in delivering care in gender affirming spaces.
  • Knowledge of health disparities and social determinants of health.
  • Knowledge of trauma informed care and harm reduction principles.

Special Requirements

  • The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye/hand coordination. Requires sitting, standing and walking for extensive periods of time. The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.
