Position Summary:

A Registered Nurse will serve as the Prenatal and PostPartum Care coordinator in Zufall's Plainsboro site. Under the direction of Zufall's Chief Medical Officer, the nurse will provide care coordination, direct patient care and services to prenatal patients enrolled in the program. Nurse will coordinate essential linkages to nutrition services, behavioral health and oral health care as well as to adjunct supports including community health workers and health education programming. Nurse will support the ob/gyn providers and collaborate with other team members to achieve excellent access and patient outcomes. The Nurse will act as alternate Director of Nursing for Plainsboro and Alternate Infection Control Coordinator for Plainsboro as needed.

Essential Job Functions:

  • Under the supervision of Zufall's Chief Medical Officer, implement an integrated comprehensive maternal and child health program, offering three points of entry: 1) Preconception Counseling 2) Primary Medical Care at Positive Pregnancy Diagnosis and 3) Postnatal Care upon Delivery. Coordinator will promote the active engagement of linkages with community partners in care, specifically with ob/gyn providers and others.
  • Working in collaboration with Zufall departments including Oral Health, Behavioral Health, Nutrition, and Special Populations, develop and implement a full range of policies and procedures for registration processes, eligibility determination, work flows, case management, and referral to central intake, specialty care, and health education programming to address various factors influencing pregnancy outcomes and pediatric clinical outcomes, including chronic disease management, healthy weight and nutrition, oral health care, domestic violence, and substance abuse.
  • Oversee the implementation of program requirements including facilitating Presumptive Eligibility for pregnant patients, the timely follow-up of prenatal patients by SPAN-NJ's Community Health Workers, and others.
  • Work alongside Zufall's Patient Centered Medical Home (PCMH) Patient Care Coordinator, Women's Health and Pediatric areas, to facilitate smooth and efficient patient flow in Zufall Plainsboro, over-seeing the case-management and referral processes.
  • Collaborate with Chief Medical Officer, Dental Director and the Zufall health care team to facilitate seamless integration of program goals, objectives, and methodology into the service delivery system, including Informational Technology system and electronic health record changes.
  • Follow Healthy Start guidelines, established protocols and procedures for the care of Prenatal and Post-partum patients, including regular assessments, acute evaluations, referrals to providers, and ensure proper documentation of all interventions.
  • Attends trainings, meetings and conferences related to patient care and other related services including Infection Control/OSHA, Patient Safety, Risk Management and QA/PI programs.  Participates with collaboratives and other improvement processes related to clinical patient services.
  • Performs all functions of Director of Nursing as needed in full compliance with the center's and Medical Department related duties                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                     

Licensure and Credentials:

  1. Registered Nurse
  2. Active State of New Jersey Registered Nurse license is required
  3. Must maintain current CPR and AED certificates
  4. Must maintain Professional Development requirements per the NJ Division of Consumer Affairs, Board of Nursing.

Education and Training:

  1. Minimum of Bachelor's of Science degree preferred or other related Bachelor's degree
  2. 3-5 years nursing experience in medical center or community health care settings
  3. At least two years experience in one of the following disciplines: Internal Medicine, Pediatrics, OB/GYN

Skills and Abilities:

Position requires a comprehensive knowledge of clinical techniques and language for all phases of Women's Health and Pediatrics. Able to work with individuals from a diverse array of social, ethnic and economic backgrounds is necessary. Must be able to work creatively and collaboratively with other health care professionals from a variety of disciplines to achieve maximal results for the Center's patients and operate efficiently and effectively within a system of integrated primary health care and across a range of diverse community-based organizations.

Typical Physical Demands:

Prolonged sitting or standing may be required. A degree of mobility, manual dexterity and hand-eye coordination for computer work, phone and other office equipment use is required. Work under stressful conditions as well as irregular hours may be required. Frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations, radiation and other conditions common to a clinical environment may be routinely encountered.

As a condition of employment, we require that all staff are fully vaccinated against flu. Therefore, if hired during flu season (typically between September-May), all new employees need to be fully vaccinated against flu and are required to provide documentation of receiving flu vaccines upon hire.
