The Case Manager provides job support, advocacy, employment skills training, planning, organization, implementation, evaluation, and other supportive services to the individuals that Powerworks Industries (PWI) serves, many of whom face multiple physical, cognitive, situational, and emotional barriers to employment.
Case Manager (Powerworks)
Powerworks – Albany, GA
Powerworks in Albany Marine Core Logistic Base is currently looking for a Case Manager to support the organization’s needs.
Powerworks Case Manager responsibilities:
- Ensures timely and accurate completion of documentation according to the prescribed schedule.
- Completes monthly Case Notes
- Ensure medical documentation is signed, and Individual Employability Evaluation (IEE) and Individualized Service Plan (ISP) are completed.
- Completes PWI Satisfaction Survey and ensures goals toward ISP Progress are met.
- Conducts a minimum of one meeting per month with each team member addressing issues as needed.
- Provides job coaching and referrals.
- Completes additional tracking reports to ensure compliance.
- Updates accommodations and documents efforts made toward ISP goals.
- Meets with prospective team members to discuss and assess program determination/eligibility.
- Assesses job readiness and educational status, and goals.
- Assesses functional limitations and accommodations for worksite placement related to physical, mental, and cognitive disabilities.
- Identifies and verifies the accommodation requested is needed and provided.
- Conducts on-site visits to provide observation and job coaching to the team members and meets with Team Leads and Supervisors to address specific concerns.
- Identifies and develops resources and appropriate support services with local agencies and organizations necessary for continued employment.
- Eliminates barriers to employment not limited to Vocation Rehabilitation Agency Work Incentive Navigator (WIN) Services, Social Security Administration, Housing Authority for subsidized housing, Behavioral Health for mental health services Metra Transit Services for bus passes, and other consultations.
Requirements:
- Requires a level of knowledge normally gained through the completion of post-graduate work
- 2-year Experience.
- Master’s degree in counseling or other related fields
