Your Role With Us:
Salem Health Medical Clinic – Boulder Creek is centrally located in Oregon’s capitol of Salem, Oregon. We’re hoping to find a Physician who is team oriented, focused on patient care, flexible, good communicator and likes to collaborate in a team environment on improvement projects and strategies.
Friendly team oriented staff, our building is single level and very spacious. We have a PMHNP, a drug and alcohol counselor and an RN case manager who can help coordinate care for those patients who need it. We have phlebotomist located in our office and the hospital is just over 2 miles away which makes it easy to access any of their services.
Background/Expertise:
- Board Certification in the applicable American Board of Medical Specialties or the American Osteopathic Board.
- Applicants who are not Board Certified must obtain board certification within five years from completion of the applicable Residency and/or Fellowship program for the privileges being requested at Salem Hospital
- Demonstrate experience and current competence in the delivery of patient care for a minimum of 60 patients of various diagnoses and procedures in each of the requested age populations below in an active clinical practice reflective of the privileges requested within the last 24 months.
- 1 to 3 years of experience in the delivery of patient care in an active clinical practice preferred.
