Pediatrician
Tapestry 360 Health (formerly known as Heartland Health Centers) is looking for a Pediatrician to provide primary health care services to children emphasizing the prevention and treatment of disease. Tap 360 Pediatricians practice independently and, as needed, collaborate and/or supervises with a Care Team comprised of nurse practitioners, physician assistants, medical assistants and other health care providers. Major Responsibilities:
- Facilitates entry of the client into the health care system by assessing the primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis).
- Documents comprehensive, accurate and continual data on patients’ records and program reports according to Tapestry 360 Health standards in an electronic health record.
- Practices within the legal and ethical guidelines established by the Medical Practice Act, the Physicians Code of Ethics, and the Specialty Standards of Practice.
- Performs other duties as necessary.
Minimum Required Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) and Board Certification or eligibility in Pediatrics required. Documentation of degree from an accredited school of medicine and pediatric residency completion from an accredited residency program required.
- Active Illinois Physician licensure, Illinois Controlled Substance License and federal Drug Enforcement Agency certification required. Board certification in Pediatrics must be completed within two years of employment. National Data Bank verification of legal claims history required.
- Basic Life Support certification required.
