Job Description
Community Health is seeking a Family Practice Physician to work at our clinic in Rutland, Vermont. Physician candidates are expected to be knowledgeable and skilled in the delivery of Primary Care. Responsibilities include, but are not limited to:
- Empower patients to take an active role in health maintenance and promote wellness behaviors
- Provide appropriate patient follow-up and communicate effectively with patients
- Refer patients to specialty care as necessary
- Provide clinical supervision to mid-level providers and nursing staff
- Participate in quality assurance activities
- Participate in State and National disease collaborative efforts

Community Health is a multi-site FQHC serving 80% of the Primary Care needs in our community (central Vermont). Our mission is to service our patients with quality, efficiency, accessibility, and equity. As the largest FQHC in northern New England, we have a large footprint that provides us with the opportunity to influence the evolving, local, state, and regional health care environment.
Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its Primary Care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
RUTLAND CLINIC:
Community Health Rutland and Rutland Express Care serve the majority of those living in and around Rutland, the county seat and population center of southern Vermont. The Rutland practice uniquely provides the community with access to Primary Care, Behavioral Health and Care Management services for the whole family. Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. Rutland Express Care, located within the same facility as Community Health Rutland, is open seven days a week for the treatment of minor illnesses and injuries for adults and children.
Downtown Rutland, the third largest city in Vermont, is the county seat and home to Rutland Regional Medical Center. The Green Mountain National Forest, Killington and Pico ski resorts are a short drive from the picturesque historic downtown area.
KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:
- Favorable completion of college, medical school, internship, residency and possibly fellowship
- Possesses and maintains current license to practice medicine in the State of Vermont required.
- Possesses and maintains current Board Certification within practicing specialty required.
- Demonstrates meeting minimum CME requirements for licensure and board certification.
- Doctors of Medicine (MD) and Doctors of Osteopathic Medicine (DO) accepted.
- Strong inpatient skill set.
- Quality-focused team player.
- Excellent organizational, time management and problem-solving skills.
- Ability to function effectively as part of a multi-disciplinary team.
- Effective oral and written communication skills.
- Commitment to providing exceptional patient care.
