Job Summary
Integrated Care Campus-Beltline Primary Care is a large busy suburban practice comprised of Family Medicine and Internal Medicine/Pediatrics providers. Located in Corewell Health West's largest Integrated Care Centers, provides the new physician with lots of resources for their patients. Co-located in the building is Urgent Care, Lab, Imaging- including MRI and CT, Neurology, Psychiatry & Behavioral Health, and Woman's Health.
This is Corewell Health West's largest Primary Care site comprised of fourteen physicians and ten Advance Practice Providers. The new addition to this practice will enjoy working in a team-based model of care with on-site social work, pharmacists, physical therapy (embedded in the practice) and RN case managers along with an exceptional staff.
Essential Functions

• Assesses and monitors the medical needs of patients/significant others.
• Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.
• Provides input into the development of policies and procedures as they related to medical practice and quality patient care.
• Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
• Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.
• Documents in a manner consistent with regulatory standards.
• Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
• Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.
  Actively works with Spectrum Health to meet financial and patient satisfaction goals.

Qualifications

• Required Bachelor's Degree

• LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or
• LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required
• CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
• CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required
