What you can expect:
The Physician for the Executive clinical Team provides cross coverage, telephone triage and general clinical care to patient panels and telephone triage for designated providers of Howard Brown Health.
How you will make an impact as clinical administrative leader:
- Answers all patient/client inquiries for assigned Providers regarding health issues. Serve as advocate for patient/client for both internal and external referrals.
- Provides education, screening and treatment, following agency clinical protocols.
- Coordinate medical care with other providers as required, schedule patient follow-up internal and external appointments as instructed by Providers.
- Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
- Evaluate all patients /clients’ ability to understand health plans, perform health education as needed, and notify appropriate Provider as necessary.
- Provide telephone triage and education to patients/clients, their significant others, and staff in a compassionate manner.
- Notifies provider regarding urgent patient needs.
- Notify providers with abnormal lab values within a reasonable period.
- Works collaboratively with the call center, nursing, and ancillary staff.
- Maintains accurate and necessary patient documentation in accordance with all regulatory bodies.
- Apply human relation skills and customer service skills in providing patient care and in establishing good rapport.
- Execute Phone Triage responsibilities: performs a systematic, comprehensive interview of the caller/patient/client to determine the urgency of the patient’s problem. Accurately documents data for all calls including, but not limited to, patient/client demographic information, chief complaint, recommendations for home treatment, and referrals to primary care provider, alternative health care provider or emergency services. Performs and documents follow-up calls with patients as necessary to help ensure positive outcomes.
- Make referrals to the appropriate level of care within an appropriate period. Schedules same-day appointments or follow-up appointments with the patient/client primary care provider or alternative health care provider as necessary
- Provide counseling, home treatment advice, and crisis intervention according to established protocols and standards of care.
- Maintain clinical competency through continuing education, recertification (including CPR), and clinical practice.
- Adhere to Howard Brown Human Resources Policies and Procedures.
- Actively participates in fulfillment of organization’s mission, vision, and core values. Adhere to OSHA standards in delivery of care to all patients.
- Participate in Quality Improvement process via data collection, evaluation of results, and implementation of recommendations.
- Perform other duties as directed.
- Evening, weekend hours are required.
- Role is required to enter the telephone call schedule
- Attends and participates in the Medical Leadership weekly meeting, clinical quality meetings as required and other leadership meetings as required.
As Clinician:
- Provides coordinated, continuous and culturally competent care.
- Prescribes a variety of treatment modalities to meet health and illness needs.
- Collaborates with social services and clinical team to assure that health teaching and counseling is provided.
- Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
- Manages and evaluates the plan of care, process and outcome and then modifies the plan as needed.
- Consulting with health team members about managing the client treatment plan.
- Following clinical practice standards (protocols) related to each health care providers role and responsibilities.
- Collaborates with other multidisciplinary professionals and community agencies to provide high quality care.
- Practices within the legal and ethical guidelines
- Documents comprehensive, accurate and continual data on clients’ records and program reports according to Organization standards.
- Adheres to professional standards as outlined by governmental bodies, private funding American sources, Organization plans/policies and Unit guidelines.
- Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance or guidance from Manager to address new issues or complex concepts affecting adherence to professional standards.

What you will bring to Howard Brown Health:
Minimum Qualifications:
- Education and/or Experience: Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a doctor of Osteopathy degree. If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate (ECFMG) certificate. Successful completion of an approved program of residency training in family practice.
- Board certification in Family Practice preferred.
- Experience in treating HIV preferred;
- Experience in providing LGBTQIA+ affirming health care preferred;
- Experience in providing culturally affirming care to people of color, communities that experience disproportionate health impacts, and people who are part of marginalized communities;
Certificates, Licenses, and Registrations:
Active Illinois Physician licensure, plus current DEA registration, documentation of a Medical Degree from an accredited school of Medicine, documentation of residency completion in a specialty area from an accredited residency program and Physician National Data Bank verification of legal claims history.
