As a Physical Therapist with Healthy Living at Home, you will provide physical therapy assessment, services, direction, and care to patients in your region. You will evaluate the patient's home environment and functional status, and participate in the development of the total plan of care.
To be successful in this role you will:
Determine priority physical therapy needs and direct physical therapy treatment
Provide physical therapy consultation to patients and their families and caregivers
Prepare and submit clinical and progress summaries based on the attainment of goals
$60 - $62 an hour
**Licensed to practice as Physical Therapist within the state Doctorate or Master (MS) degree in physical therapy; Minimum of one (1) year of experience as a Physical Therapist and current CPR; Must maintain a valid driver's license, active auto insurance meeting state minimum requirements and an operational vehicle.
