Physical Therapist, Part Time *** Assisted Living Facility ***

UNH Therapy Services is seeking a Physical Therapist to join our team in Sacramento, CA.

Job Description:

• Provides patient care and treatment in a clinical setting
• Assists patients in all activities of daily living
• Performs performance assessments and develops treatment plans
• Develops and implements patient treatment plans; provides treatment instructions to patient and family members
• Conducts patient treatments per state licensing requirements
• Provides accurate documentation of patient care and treatment
• Reviews patient medical history to identify and treat chronic conditions
• Provides instruction on the use of equipment (wheelchair)
• Prepares materials and supplies needed for treatments
• Adheres to all OSHA safety protocols and regulations
• Ensures that all necessary paperwork is completed for each patient visit
• Ensures that all necessary paperwork is completed for each family member visit(s) per state licensing requirements
• Performs other duties as assigned by supervisor or manager

Requirements:

• Master’s Degree in Physical Therapy from an accredited college or university required; Doctorate in Physical Therapy from an approved school required. Degree in Physical Therapy from an approved school will be considered for entry level positions. Degree in Physical Therapy from an approved school will be considered for advanced degree positions. Certification as a Registered Physical Therapist (RPT) is preferred. Must have valid driver’s license, auto insurance, car insurance, and reliable transportation. Must have access to your own vehicle. Must be able to pass a background check and drug screen. Must not have been convicted of any crime. Must not have been found by state licensing authorities to be incompetent or unqualified in the practice of physical therapy. Must not have been found by state licensing authorities to be guilty of malpractice or which required immediate suspension or termination under any state licensing authority. Must not have been found by state licensing authorities to have violated any company policies or procedures related to patient care, safety, infection control, or any other critical areas of business operations. Must be able to speak, read, write, and understand English. Spanish speaking Physical Therapist required.

Job Type: Part-time

Pay: $55.00 - $70.00 per hour

Expected hours: 10 – 24 per week
