Form of workCommunity Nursing Services, "CNS" is looking for a PRN Physical Therapist to work in the greater Provo/Orem/Utah County area. This position could grow into a Full Time opportunity as well.
Compensation is $65 to $95 an visit , not hourly (Zip didn't give a by visit option).

JOB SUMMARY
Our Physical Therapists’ work;
This position is accountable to evaluate and treat all patients referred for Physical Therapy services, to assist in their return to optimal functional abilities, and to prevent additional disability. This includes communication with physicians and other health care personnel to assist in providing quality patient care.
Some of the job responsibilities may include but are not limited to:
- Respect others by treating them the way we want to be treated.

- Ability to prioritize and coordinate patients needs.
- Able to plan, develop, organize and implement successful Therapy plans.
- Ensure excellent communication with patients and families
- Assist with improving our current processes and PT Team
REQUIREMENTS
- Ability to work independently and cooperatively as a team member
- Ability to be accurate, concise, and detail oriented
- Caring and compassionate
- Physical Abilities: Lift 50-100lbs
- HHC and or Hospice Experience Preferred
Licenses/Certifications: State License-Physical Therapist- Utah (Physical Therapist Registered)
