Community Nursing Services, "CNS," looking for a full-time Physical Therapist to work in the greater Layton area.
Pay - Is BY VISIT- $55 - $65 and $95 for Admissions
Schedule: Monday - Friday
Hours: 8am - 5pm

JOB SUMMARY
Our Physical Therapists’ work
This position is accountable to evaluate and treat all patients referred for Physical Therapy services, to assist in their return to optimal functional abilities, and to prevent additional disability. This includes communication with physicians and other health care personnel to assist in providing quality patient care.
Some of the job responsibilities may include but are not limited to:
- Respect others by treating them the way we want to be treated.
- Demonstrate kindness, compassion, honesty, and integrity at all times.
- Ability to prioritize and coordinate the needs of multiple patients.
- Able to plan, develop, organize and implement successful Therapy plans.
- Ensure excellent communication with patients and families
- Assist with improving our current processes and PT Team
REQUIREMENTS
- Ability to work independently and cooperatively as a team member
- Ability to be accurate, concise, and detail oriented
- Caring and compassionate
- Physical Abilities: Lift 50-100lbs
- HHC and or Hospice Experience Preferred
Licenses/Certifications: State License-Physical Therapist- Utah (Physical Therapist Registered)
