Physical Therapist

Elite Health Care is a vibrant, nurse owned Home Health Agency that is passionate about delivering the best care to our patients. We are seeking a Physical Therapist(PT) with a positive and professional attitude to join our team. We use online documentation and will provide a username and password for you.

PT Evaluations are $140.00 per visit.

Primary Responsibilities:

• Perform physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.
• Implements teaching appropriate to the client's needs and within the context of his/her environment and value system.
• Assist in the development, implementation, and evaluation of the on-going quality improvement process.
• Demonstrates knowledge of safety/infection control practices by compliance with agency policies and procedures.
• Performs in-home supervision of home health aides and physical therapy assistants as directed and according to agency policy.
• Performs other related duties and responsibilities as deemed necessary.

Necessary Qualifications:

• Graduate of a Physical Therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
• Current license/registration as a Physical Therapist in the state of California.
• Minimum of one (1) year's experience in a health care facility. Home care experience preferred.
• CPR certified.
• Strong verbal and written communication skills and good interpersonal skills.
• Knowledgeable in the principles and practices of physical therapy.
• Current driver’s license, good driving record, and reliable transportation.

This is a per diem position with the potential for a full time position.
