Job Description Summary
Positive patient identification:Verify positive patient and sample identification, according to hospital policy and the “Patient Identification Procedure" policy in the Phlebotomy SOP Edit
a)Verify and ensure positive patient identification in a confidential manner 100% of the time
b) Follow established protocol for missing, illegible, or inaccurate wristbands (inpatient)
c) Use two (2) identifiers for patient verification (both inpatient and outpatient) to include full name and MRN/Date of birth
d) Ensure no more than 1 mislabel or unlabeled specimen per year
e) Print patient labels, via Epic, at the bedside, do not pre-print labels
f) Label samples, at the bedside, immediately following specimen collection
g) Complete all required documentation prior to submitting the specimen to the lab
h) Document collection information to include VP number/identification of collector in medical record
Specimen Collection:
Collect venous and capillary specimens from pediatric, adult and geriatric patient populations using aseptic technique and according to hospital policy and the “Collection of Blood Specimens by Venipuncture” and “Collection of Blood Specimen by Skin Puncture” policies in the Phlebotomy SOP
Ensure correct container and transportation requirements, sufficient quantity, and apply adequate pressure following puncture to prevent:
a) Hematoma formation, Hemolysis of sample, Clotting (when inappropriate for testing), QNS samples
b) Use appropriate tools for blood collection needs (i.e., finger lancets vs. heel lancets, hypodermic needle vs. butterfly
c) Verify removal of tourniquet
d) Follow all precautionary isolation signs 100% of the time
e) Seek assistance after two (2) unsuccessful attempts
f) Complete labels and requisitions in accordance with documentation requirements.
g) Notify appropriate personnel of relevant information; inability to collect ordered specimens, change in patient status, timing synchronization issues, etc.
h) Complete logs and documentation in Epic, unit log book, and the lab, as needed.
i) Conduct specimen collection rounds on time.
j) Monitors worklist throughout shift, clears worklist before leaving shift and communicates needs to next shift.
k) Maintains blood culture contamination rate below 2.5%
LIS functions:
Perform “basic” Laboratory Information System (LIS) and Epic functions to ensure operational efficiency.
