Form of workThe Pharmacy Technician position is responsible for providing supportive Pharmacy Services within the Value Based Service Organization clinically integrated network. The position is responsible for assisting in the review of high-cost patient medications, patients with multiple medications and medication adherence issues to close gaps around medication management. The pharmacy tech will be well versed in reporting build, pull and analysis to determine best areas of focus for patients and providers. This position is responsible for providing customer service to internal and external customers, working under the operational supervision of the pharmacist. The majority of tasks and responsibilities are related to medication management.
The Pharmacy Technician will outreach to assigned patients to collect information requested by the pharmacist and schedule medication therapy management appointments with the pharmacist. The technician will work collaboratively with a multi-disciplinary care team which includes nurses, social workers and providers and help with coordination of care with patients. This technician will also help develop presentations, charts and workflows in collaboration with the pharmacist.
Qualifications:
- 1 year Pharmacy Technician experience preferred
- Children's Hospital Pharmacy and/ or Value Based Care program experience preferred but not required
- Medication Therapy Management experience/certification preferred but not required.
