The Pharmacy Technician position is responsible for providing supportive Pharmacy Services within the Value Based Service Organization clinically integrated network. The position is responsible for assisting in the review of high-cost patient medications, patients with multiple medications and medication adherence issues to close gaps around medication management. The pharmacy tech will be well versed in reporting build, pull and analysis to determine best areas of focus for patients and providers. This position is responsible for providing customer service to internal and external customers, working under the operational supervision of the pharmacist. The majority of tasks and responsibilities are related to medication management.
The Pharmacy Technician will outreach to assigned patients to collect information requested by the pharmacist and schedule medication therapy management appointments with the pharmacist. The technician will work collaboratively with a multi-disciplinary care team which includes nurses, social workers and providers and help with coordination of care with patients. This technician will also help develop presentations, charts and workflows in collaboration with the pharmacist.
Qualifications:
- 1 year Pharmacy Technician experience preferred
- Children's Hospital Pharmacy and/ or Value Based Care program experience preferred but not required
- Medication Therapy Management experience/certification preferred but not required.
Essential Functions Direct telephonic outreach to patients, providers, pharmacies to collect information, schedule appointments, coordinate care and provide follow-up support under the supervision of a pharmacist. Works collaboratively with the pharmacy team to review and manipulate data in various platforms such as Excel, electronic medical records and/or other population health platforms. Works collaboratively with the pharmacy team to develop presentations, diagrams/charts and workflows. Demonstrates familiarity with the general duties and responsibilities of a Pharmacy technician including knowledge, competency and understanding of the position to which he/she is assigned which may include: PBM proficiency, report analysis, outreach to families and/or providers to help with medication management, coordination of appropriate medication refills and education, assistance with medication related problems around cost, transportation, medication delivery and medication questions. Participates in departmental program development and Continuous Improvement (CI) activities. Performs other related duties as assigned by authorized personnel to provide departmental flexibility to patient care needs including those for emergency situations. Carries out rules and regulations pertaining to the professional policies of the department and ensures compliance with State and federal regulations. Able to maintain punctual attendance consistent with Nemours policies, regulatory, federal, state and local standards.
