PHARMACIST II
Hilo Medical Center, Pharmacy Department | Hilo, Hawai’I | 100% Full Time Equivalent with Benefits
Duties Summary: Provide comprehensive and individualized pharmaceutical care services including but not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
Examples of Duties:
- Provide pharmaceutical services throughout all hospitals and collaborates with healthcare providers as an active part of a multidisciplinary team.
- Reviews patient profile/chart and drug orders/prescriptions to provide accurate, safe, timely and appropriate medication therapy and to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interaction, failure to receive drugs, untreated indications, medication without an indication and treatment failures.
- Provides discharge medication review, reconciliation, and counseling as appropriate.
- Assists the Director of Pharmacy with the development, implementation and facilitation of the Pharmacy Unit's qualify improvement plans and activities. Tracks and evaluates the Pharmacy Unit's projects for operational, quality and financial efficiency.
- Assists the Staff Pharmacists in the performance of their duties (i.e interpreting orders/prescriptions; compounding medications; reviewing patient records and orders for drug therapy and recommending and initiating changes as appropriate to ensure an effective, safe, and cost-effective drug utilization while maintaining and enhancing patient care.
- Develops and implements a staff teaching and development program to address traiing oppurtunities of the Pharmacy Unit. Provides training in the identified areas of opportunity as well as teaching on new techniques, equipment, and research findings.
- Provides medication and practice-related education/training and actively serves as preceptor for Doctor of Pharmacy students. Maintains proficiency in preceptor roles and emulates education/mentorship skills.
Knowledge and Abilities Required:
Knowledge of: The principles and practices of professional pharmacy and their application to the operation of a pharmacy; specialized sciences directly applicable to pharmacy, such as pharmacokinetics, pharmacology, pharmacognosy, posology, toxicology and therapeutics; pharmaceutical instruments and techniques used in compounding.
Ability to: Apply acquired knowledge to pharmacy processes such as reading and interpreting prescriptions, instructions and procedures; keep records and prepare reports; establish and maintain effective working relationship with medical and administrative personnel within the facility.
Education Requirement:
Graduation from an accredited college or university with a baccalaureate or Pharm.D degree in pharmacy.
License Requirement: A valid license with the State of Hawaii as a Pharmacist at the time of appointment.
Professional Pharmacist Experience: Applicants must possess full-time work experience as a licensed pharmacist in the amounts listed below:
Pharmacist II: 1 year
Non-Qualifying Experience:
Pre-license experience performed to meet licensing requirements is not qualifying to meet professional pharmacist experience requirements.
Supervisory Aptitude:
Applicants for Pharmacist I, II and III must possess supervisory aptitude. Supervisory aptitude is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader, previous work experience in a position that demonstrates or performs supervisory functions or in similar work in which opportunities for demonstrating supervisory capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.
Quality of Experience:
Possession of the required number of years of experience will not in itself be accepted as proof of qualification for a position. The applicant's overall experience must have been of such scope and level of responsibility as to conclusively demonstrate that he/she has the ability to perform the duties of the position for which he/she is being considered.

Offers of employment will be contingent on successfully passing a pre-employment examination, which includes drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen. The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.

Shift DetailsMust be available to work shifts, weekends and holidays as necessary based on hospital operations. Services are provided 24/7.
