The Clinical Pharmacist - Oncology will provide collaborative pharmaceutical care to patients receiving antineoplastic therapy or palliative care therapy in the setting of malignancy. Additionally, the pharmacist will work in collaboration with pharmacy leadership, the hematology/oncology providers, and infusion centers to monitor chemotherapeutic utilization, supportive drug therapy utilization, adverse drug reactions, therapeutic regimens, and patient and provider safety.
ESSENTIAL FUNCTIONS:
Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.
Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).
Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
Complies with Network and departmental policies regarding attendance and dress code.
Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
Develop service line clinical pathways as necessary.
Monitors effectiveness and staff compliance with clinical programs in coordination with the Clinical Coordinator for Network Pharmacy Services.
Collaborates with the Clinical Coordinator in developing and implementing clinical activities for pharmacy staff.
Educates medical residents/interns, pharmacy residents, case managers, nurses, physicians, pharmacists, and students.
Assists in developing pharmacy policies and procedures related to anti-neoplastic medications, treatment of malignancy, safe handling of hazardous medication, and drug utilization.
Mentors pharmacists, pharmacy residents, and students.
Interacts with health care professionals; participates in medical rounds as needed.
Participates in order entry and related monitoring activities, including:
  • Allergy and allergy history clarifications
  • Medication history
  • Drug selection
  • Drug appropriateness for condition
  • Interactions including drug/drug, drug/food, and drug/lab
  • Renal and hepatic function
  • Culture and sensitivity
  • INR/PTT/PT and other related lab values
  • TPN formulation
  • IV to PO streamlining
  • Pain management
  • Pharmacoeconomics

Implements and coordinates investigational protocols as required and in cooperation with the investigational drug service. Provides drug information. Attends committee meetings as required. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to age of the patient treated. Completes and maintains departmental competencies as related to the position.
OTHER FUNCTIONS:
As designated by Pharmacy Management Other related duties as assigned.
PHYSICIAL AND SENSORY REQUIREMENTS:
Sitting for up to 8 hours per day and up to three consecutive hours. Standing to for up to 8 hours per day and up to three consecutive hours. Walking for up to 8 hours per day and up to 30 consecutive minutes. Continuously fingering; frequently handling, firm grasping, twisting, and turning. Frequently reaching above shoulder level. Frequently looking up. Occasionally lifting up to 20 pounds and carrying up to 35 pounds and pushing up to 200 pounds. Occasionally stooping, bending, squatting, crouching, and kneeling. Rarely crawling and climbing. Hearing as it relates to normal conversation, low and high frequencies. Seeing as it relates to general vision, near vision, far vision, depth perception, peripheral vision, color vision, and monotony.
POTENTIAL ON-THE-JOB RISKS:
Cuts and punctures - recapping of devices may be required
Chemical hazards - eye protection, gloves and gown required, chemo spill kit available
Biological hazards - adherence to hospital infection control policies
Employees who work in patient care areas where they may have potential exposure to patients with suspected or proven tuberculosis (TB) must have the ability to wear a particulate respirator and be fit-tested in compliance with the current recommendations from the CDC (enforced by OSHA) or must provide physician documentation as to the inability to wear a particulate respirator.
SPECIFIC PROTECTIVE EQUIPMENT AVAILABLE:
Gowns, gloves, masks, eye protection, closed system transfer device, biological safety cabinet
MOST COMPLEX DUTY:
Accurately assessing patients medication therapy to allow for appropriate recommendations and actions.
SUPERVISION (RECEIVED BY AND/OR GIVEN TO):
Received: Pharmacy Management - Clinical coordinator, site director of pharmacy, network director of pharmacy; Senior Hospital Leadership
Given: Pharmacists, technicians, pharmacy residents, interns, students, volunteers
COMMUNICATIONS:
Communicates with physicians, nurses, and other hospital personnel. Strong verbal, written, and customer service skills.
ADDITIONAL REQUIREMENTS:
MINIMUM QUALIFICATIONS
EDUCATION:
PharmD. Required.
LICENSURE:
Registered in Pennsylvania or completion of Boards with pending licensure may be accepted. State mandated continuing education.
TRAINING AND EXPERIENCE:
ASHP accredited PGY-1 Residency required. ASHP accredited PGY-2 residency in Oncology or equivalent experience required.
WORK SCHEDULE:
Normally day shift Monday through Friday. Additional times as needed.
