Essential Duties and Responsibilities:

1.) Participates in pharmacy clinical services activities which include but are not limited to:
  • Providing drug information, clinical consultation, and clarifications of drug therapy to healthcare professionals.
  • Detecting, assessing and reporting adverse drug reactions and medication errors.
  • Accurately documenting pertinent clinical interventions and cost-saving opportunities.
  • Participates in Pharmacokinetic Dosing Service and/or Antibiotic Stewardship: monitors and interprets appropriate clinical data, calculates initial doses, orders levels and adjusts doses when necessary.
  • Reviews and interprets culture and susceptibility data for appropriateness and recommends rational alternatives when available.
  • Anticoagulation Service: monitors and interprets appropriate clinical data, suggests appropriate dose adjustments as necessary, provides information on appropriate reversal as required and doses warfarin when requested by prescriber.
  • Adjusts renal dosing of targeted drugs based on creatinine clearance and converts intravenous to oral therapy (IV to PO conversions) when appropriate, per department protocols.
  • Upholds the pharmacy formulary, therapeutic substitution protocols, and other specific drug protocols as required.
  • Serves as a Pharmacy representative: attends interdisciplinary patient care rounds; participates in Nutrition Support rounds and monitoring of patients requiring parenteral nutrition; and/or provides in-service education to patients, healthcare practitioners, or other community groups.
2.) Staff Development:
  • Provides formal/informal educational materials, services, and programs to pharmacy staff, hospital staff and/or community organizations.
  • Assists the Pharmacy Administration in implementing clinical programs for pharmacy staff involvement and provides clinical assistance outside the department upon request.
3.) System Functions:
  • Actively participates in committees, including but not limited to: Pharmacy and Therapeutics Committee, Antimicrobial Stewardship, Medication Safety Committee, and others as determined by the Preceptor/Residency Program Director.
  • Provides assistance in developing, implementing and performing analysis of drug usage evaluations.
  • Assists in maintenance of the drug formulary through preparation of drug monographs and therapeutic drug substitution documents for presentation to Pharmacy and Therapeutics. Provides education to pharmacy staff of formulary changes. Seeks feedback/input from the pharmacy staff, physicians, nursing staff and patients and other health care members as appropriate in the preparation for decision making.
4.) Precepts, develops and serves as a mentor and role model to pharmacy students through clerkships, internships, and externship programs.
5.) Performs all functions of a registered pharmacist and coordinates activities with pharmacy supervisor/manager.
  • Participates in the distributive function and/or clinical services during staffing requirements.
  • Reviews, interprets, and processes medication orders (prescriptions) and screens for completeness, accuracy, and clinical appropriateness to assure that correct medications, dosages, dosage intervals and route of administration have been prescribed.
  • Effectively utilizes the patient medication profile, pertinent lab data, or other drug monitoring parameters to screen for drug allergies, significant drug-drug / drug-food interactions, contraindications, therapeutic duplications, and proper dosage with respect to age and other patient-specific parameters.
6.) Participate in pharmacy performance improvement projects which enhance medication use processes, reduce medication errors and advance patient safety, reduce costs, and eliminate waste such as medication safety and pharmacy performance improvement activities.
7.) Performs other duties as assigned.
Education and/or Experience:
  • Pharmacy Residents must have a Doctorate of Pharmacy (PharmD) degree from a College of Pharmacy accredited by the American Association of Colleges of Pharmacy (ACPE).
  • The applicant must be a citizen of the U.S.A., or hold a visa allowing for the completion of the residency year.
  • No prior experience is required, however, the resident must desire to be a highly motivated pharmacist wanting advanced education and training that leads to an enhanced level of professional pharmacy practice.
Certificates, Licenses, Registrations:
  • Pharmacy Residents must obtain their Texas State Board of Pharmacy License within 90 days of starting the program and be in good standing throughout the entire program duration.
