Contracting with @Home Therapy as a provider enables you to work in the area that you choose and on your own hours. @Home Therapy offers EMR training and orientation to new Physical Therapists as well as ongoing clinical and administrative support.
We seek a Physical Therapist with the following qualifications:

Contracting with @Home Therapy as a provider enables you to work in the area that you choose and on your own hours. @Home Therapy offers EMR training and orientation to new Physical Therapists as well as ongoing clinical and administrative support.
We seek a Physical Therapist with the following qualifications:
- A valid Physical Therapist License and Registration in New York are required.
- Experience in adult Physical Rehabilitation is required.
- Excellent communication skills are required.
- Familiarity with EMR software is a plus.
- Homecare Experience a plus.
Responsibilities of the Physical Therapist include:
- Communicate with the office staff regarding your patients.
- Schedule your PT visits directly with your patients.
- Evaluate and develop a plan of care according to the patient's prior and current functionality.
- Provide follow-up visits in accordance with your plan of care and the patient's progress toward goals.
- Document your visits in the web-based EMR in accordance with Medicare's guidelines.
- Maintain active credentials and perform all visits within HIPAA and company compliance.
