Summary
The Family Health Center (FHC) Mobile Crisis Team provides mental health engagement, intervention, and follow-up support to the citizens and clients of the community as well as be a liaison to treatment providers. The team may offer a range of services, including: 1.) Assessment 2.) Crisis intervention 3.) Supportive counseling 4.) Information and referrals to FHC community-based mental health services.
FHC is looking for a passionate, caring, sensitive, flexible, and dedicated Peer Support Specialist (PSS) for the Mobile Crisis Team who will provide mobile crisis services in the community and facilitate follow-up services to secure safe disposition for clients of all ages in immediate crisis. The PSS must demonstrate knowledge of identification of needs, symptoms, and behaviors of individuals who FHC serves. The PSS will work within a family's home, other community agencies, and will transport clients when appropriate.
Essential Job Duties
  • Provide crisis services to individuals within the community.
  • Provide collaboration and advocacy for positive outcomes for individuals in a crisis situation.
  • Develop and maintain positive relationships with both internal and external customers.
  • Remain current with all required training and certifications.
  • Perform triage of telephone contacts and on-site crisis situations to distinguish between routine, urgent, and emergent situations, complaints, and difficulties.
  • Provide crisis support and intervention either by phone or in person to individuals in psychiatric crisis.
  • Facilitate safe and adequate dispositions as dictated by the nature of the individual's difficulties.
  • Identifies appropriate family and community resources and gathers appropriate release documents to arrive at safe disposition of client in crisis.
  • Perform all necessary clinical documentation in a time frame that is consistent with the expectation as documented in the Family Health Center policies and procedures.
  • Consults with other disciplines, Family Health Center departments, colleagues, administrators, law enforcement, first responders, etc. in an effort to communicate, coordinate, and deliver quality, effective services to individuals in crisis.
  • Provide transportation for clients in crisis when appropriate to ensure safe disposition or attendance at follow-up services.
  • Other duties as assigned.
Minimum Qualifications
Hight School Diploma or G.E.D. preferred.
Must be at least 23 years-of-age with at least one year of recovery, three years preferred.
Must have a history of lived experience with substance use or mental illness, and/or experience with a family member with substance use or mental illness history.
