Job Description:
The IHH Peer Support Specialist functions collaboratively with the IHH Team, community providers, local hospitals, medical providers, and other disciplines to provide and enhance client care. The IHH Peer Support Specialist is responsible for quality client care and care coordination services to high-risk members.

- The IHH Peer Support Specialist adheres to all FAC policies and ethical standards and proactively strives to maintain and be an active part of the IHH team.
Job Responsibilities
- Works within a team to efficiently lead and effectively support and advocate for clients working with FAC’s Integrated Health Homes (IHH).
- Assist clients in completing patient assistance and other public assistance forms of creative problem-solving.
- Complies with IHH requirements for federally funded healthcare programs (Medicare and Medicaid) regarding fraud, waste, and abuse. Brings any questions or concerns regarding compliance to the immediate attention of the IHH supervisor.
- Maintain an active workload by Managed Care and Iowa Medicaid’s IHH performance standards, striving to achieve benchmark goals and IHH yearly measures chosen by MCOs.
- Provides support and advocacy for families during medical, dental, vision, behavioral health, and academic appointments
Demonstrates flexibility, perspective, and positive cooperation. - Develops personalized health goals, discover coping skills, access community resources, and coordinate care.
- Effectively using technology such as computers, cell phones, and software.
- Documents interactions and other required reports in an electronic health record.
- Works within a team with the IHH supervisor to ensure regulatory compliance standards are met (HIPAA, OSHA, Iowa Code, COA) and any issues are followed and reported as appropriate
- Works with the family to prevent gaps in care, self-management, and skill development.
Job Type: Full-time
Pay: $12.00-16.00 per hour
