Peer Support Specialist Fisher Project Case Manager

Objective:

The Peer Support SpecialistCase Manager is a full-time position that will work under the Fisher Project
Program Director to provide one-on-one case management to clients who are referred to the Fisher
Project through the hospital or other agencies. Clients include those who have attempted suicide or have
been identified with chronic suicide ideation.

Responsibilities:

1. Participate in continuing education and training related to suicide prevention and any other
   pertinent information.
2. Learn all specific case management protocols for the Fisher Project.
3. Maintain Peer Support Specialists' credentials and stay compliant with the LAMHA Code of Ethics.
4. Maintain contact with clients as dictated in protocols. Protocols include screening, assessments,
   safety planning, and discharge planning.
5. Chart client assessments and progress in the program.
6. Keep all client charts in working order and up to date.
7. Identify community resources for clients and families participating in the Fisher Project.
8. Track data as determined by funder requirements.
9. Meet weekly with the ERSP Project Director.
10. Participate in LAMHA meetings as needed.
11. Perform other duties as identified and assigned by the Fisher Project Program Director and the
    Executive Director of LAMHA.
12. Handles the job's physical demands, including standing, walking, climbing, kneeling,
    bending, and lifting at least 25 pounds. (For example, a box of copy paper weighs
    approximately 25 lbs.)

Accountability:

The PSS Case Worker is accountable to the Fisher Project Program Director.

Qualifications:

The PSS Case managers must be a certified Peer Support Specialist in good standing.
Special consideration will be given to any PSS who has gone through a national or advanced certification.
Advanced degrees are preferred but not required. Candidates should have no less than 2 years of case
management or other behavioral health setting experience.
