Our clinicians provide evidence-based interventions via the Collaborative Care Model to promote symptom reduction and optimal patient outcomes. You are a core member of a Collaborative Care team that involves the patient's Family Practice provider, psychiatric consultant, and other Behavioral Health providers.
The is not the typical psychotherapy model. We provide brief interventions using evidence-based techniques such as Problem Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation in Primary Care, Family Practice, and OBGYN settings.
Responsibilities
- Screens and assesses patients for common Behavioral Health and substance abuse disorders using PHQ9, GAD7, or other tools as required and provides basic education and available treatment options.
- Use brief solution-focused and problem-solving check-ins typically ranging from 5 -20 minutes, by phone or video. The types of visits will vary. Our patients can have mild to moderate anxiety and/or depression. Some may have higher acuity at times.
- Documents patient engagement and follow-up care in the EMR. Tracks patient follow-up and clinical outcomes.
- Documents patient progress and treatment recommendations in a registry so they can be easily shared with PCPs, psychiatric consultants, and other treating providers. Utilize the registry to identify and re-engage patients.
- Facilitates referrals for clinically indicated services outside the primary care clinic such as social services, housing assistance, vocational rehabilitation, Behavioral Health specialty care, or substance abuse treatment. Provides referrals to evidence-based psychosocial treatments as clinically indicated.
- Facilitates treatment plan changes for patients who are not improving as expected in consultation with PCP and psychiatric consultant. This may include changes in medications or psychosocial treatments with appropriate referrals for additional services.
- Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient's primary care provider.
Successful Applicants will have
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation).
- Experienced utilizing Behavioral Health tools (PHQ9, GAD7, CSSRS, etc.).
- Familiarity with brief, structured intervention techniques using evidenced-based techniques.
- Highly proficient in using many different platforms concurrently, including EMRs. Comfortable switching between screens throughout the day, and the ability to quickly learn new technology.

Qualifications
- Master's degree in a Behavioral Health related field
- New graduates welcome!
- Experience working with the pediatric population (ages 6 and up)
Schedule
- 100% Remote
- Full-time 40 hours/weekly
