JOB SUMMARY:
The private duty pediatric RN will be responsible for providing professional nursing care to patients in their home by assessing, implementing, and evaluating home nursing care needs on an individual and continuing basis.
Client: Male, 5 years old
Location: Merrick, Long Island
We are looking for 2-3 RNs with Trach/Vent Pediatric experience to work with us doing SHIFT work ranging from 10-12 hours per day / 2-4 shifts per week.

QUALIFICATIONS:
- New York State RN License, BSN Preferred.
- Current BCLS/ACLS and Infection Control certification.
- Valid NYS Driver’s License and access to a car is a plus.
- Minimum of 3-5 years high acuity pediatric nursing care.
- Minimum of 2-year experience with complex pediatric in-home patients.
- Minimum of 1-year experience in home care preferred.
- Strong independence with assessment, documentation, and clinical skills.
- Strong verbal and written communication skills are required.
- Utilization of critical thinking skills to prioritize and manage patients’ needs and expectations.

RESPONSIBILITIES:
- Understands and adheres to all applicable regulations, standards, and federal, state, and local laws and established Agency policies and procedures.
- Complete physical assessments and monitor vital signs during each client visit.
- Report all concerning health changes to the internal care team and relevant parties.
- Administer on-going care for each patient and provide necessary follow up.
- Dispense medication, monitor vital signs, and provide medication administration as prescribed.
- Establish and maintain professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care.
- Monitor well-being of patients while ensuring the delivery of dependable, high-quality care.
- Document patient care in a timely manner and in accordance with agency, state, and federal guidelines in the Company’s electronic medical record (EMR).
- Communicate regularly with members of the clinical team.
- Communicate, instruct, and educate patients/family on potential procedures, healthcare provider instructions, referrals to specialists.
- Abides by the organization's compliance program, policy and procedures, and requirements.
