JOB DESCRIPTION
Summary/Objective
· The Patient Care Navigator assumes the responsibility of coordination of care and staffing functions in accordance with state, federal, and local regulations.
Essential Functions:
· Responsible for scheduling the Agency patient/client visits.
· Provides staffing feedback to DON when needed.
· Assess, plan, and coordinate changes in patient/client care services.
· Responsible for assisting to coordinate needs of all patients/clients of the Agency.
· Assigns and verifies that all nurses, aides, and other disciplines are in compliance with documentation (timely submission and QA), and that the daily schedule is being followed. Reports any deficiencies to DON.
· Maintains and updates patient/client information to enhance highest quality patient/client care.
· Consults with all members of the treatment team and DON on regular basis to coordinate quality, goal-oriented home health care.
· Assists with and serves as liaison between patients/clients, caregiver(s), physicians, physician’s office staff, Agency administrative staff, and contracted services.
· Provides clinical and staff feedback as needed.
· Assists the DON in tracking the admission/certification and recertification process.
· Establishes, monitors, and evaluates productivity standards by conducting chart audits for completion of all appropriate documentation on a regular basis. (Authorized versus staffed hours/visits, order management tracking, recert paperwork tracking, referral data entry, admission, and discharge audits as well as other reports as requested)
· Serves as assistant of quality assurance/quality improvement and assures general compliance with all federal and state standards.
· Consults with all members of administration and professional staff on a regular basis to coordinate appropriate goal oriented, quality nursing and medical care.
· Assists in marketing the Agency programs through community in-services and presentations, meet and greet with families and or nurses, announcements, and interviews, or whenever opportunity arises.
· Enhances professional growth and development through participation in education programs, staff in-services, and workshops. Performs other functions as assigned by DON

Work Environment
· Works in a routine office environment. The noise level may be moderately high. The ability to work a flexible schedule and extended hours.
Physical Demands
· Prolonged sitting and some standing required. Occasional need to lift, pull, carry, and push items weighing up to fifty pounds. Frequent need to stoop, kneel, and reach while accessing files.
Requires working under some stressful conditions to meet deadlines, and employer/employee needs. Requires hand-eye coordination and manual dexterity. Requires excellent problem-solving skills.
Position Type/Expected Hours of Work
· This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m.
Travel
· The ability to travel locally with some exposure to inclement weather. Reliable transportation and valid auto liability insurance.
Preferred Education and Experience
· High school graduate (college preferred).
· Licensed Vocational Nurse (LVN) License: One year of experience strongly preferred.
