Personal Care Assistant

We are looking for a new team member who has strong professionalism, integrity, dedication, technical abilities, and excellent people and communication skills to assist the Office Supervisor in daily operations in order to help seniors with their activities of daily living (ADLs) and report any changes in health. This position will work closely with the Field Supervisor and other support staff to ensure clients receive the best possible experience and care. This is a full time, hourly (non-exempt), often “on-call as needed” position to assist senior community members in their home.  Over the years, PHS has established a diverse client base offering an excellent opportunity to gain experience in a wide variety of client needs.  

100% - Field Tasks

• Follow a client’s care plan and documents ADL’s as required
• Observe, report, and communicate any changes in the client's condition
• Assist with Personal Care (hygiene, dressing, etc.)
• Help clients with the self-administration of medications
• Perform light housekeeping, laundry, and assist with meal preparation
• Transport client to appointments and daily errands (varies by client)

It is understood that not all responsibilities are listed in this offer and your immediate supervisor may add responsibilities based on fieldwork demand, compliance modifications, staff needs, etc.

Pre-Employment Requirements

• Be at least 21 years old

• 2 Part TB test
• Wellness Check / Physical 
• Valid Driver License and Auto Insurance 
• CPR/AED
• Fingerprinting (obtained upon hire)

Required Qualifications

• High school diploma
• Honesty
• Punctuality, being on time
• Strong Communication Skills
• Strong desire to learn and grow
• Show a high level of efficiency in clinical skills through a competency checklist and skills assessment

Preferred Qualifications

• 1-5 years of relevant in-home care or skilled facility experience or equivalent
