ESSENTIAL DUTIES ANDRESPONSIBILITIES
- Collaborates with Senior Director of Outreach,Vice President of Patient Navigation and applicable leadership in The System,maintenance and continued development of Patient Transitions to The System andcommunity outreach programs
- Conducts nursing assessments and observations ofpatients referred for admission and similar nursing visits for currenthospitalized Health System patients
- Conducts high level presentations andfacilitates meetings with key referral source representatives
- Represents The System in the field in order tofacilitate referrals and admissions, streamlines the referral process, andconducts referral visits and patient assessments
- Assists with obtaining referral documentation andphysician orders when necessary
- Communicates with applicable system locationsregarding Patient Transitions
- Coordinates new and existing relationshipbuilding with major and minor referral sources
- Assists The System in constructing strategicinitiatives designed to explain and educate providers and the community about whatThe System provides
- Establishes and maintains professionalrelationships with all referral sources, including but not limited to casemanagers, discharge planners, social workers, physicians and physicianpractices, and any other community or third party referral sources
- Maintains thorough knowledge of market/communityin which PTD services in regards to potential new referral sources, hospitaldischarge trends, and any other relevant demographic information relevant tothe continued growth of The System
- Maintains awareness of competitors in the marketand develops profiles
- Maintains working knowledge and technical skillfor the use of hospital referral portal platforms
- Meets established goals for referrals,admissions, conversion ratios, and other quality metrics determined byleadership
- Reviews monthly analytic data. Tracks information to determine markettrends, and develops action plans based on data relative to key performanceindicators
MINIMUMQUALIFICATIONS
- Have an RN license with at least oneyear related experience in one of the following: marketing healthcare products or services,long-term care, inpatient rehabilitation or health care community relations
- Valid Georgia Driver's License with the abilityto regularly travel throughout assigned region and as necessary
- Excellent customer service, interpersonal andoral/written communication skills
- Knowledge of long-term care admissionrequirements and Medicare, Medicaid, Private Pay and Third Party reimbursement
- Ability to multi-task and function with littledirection
