THIS IS A GRANT-FUNDED POSITION LOCATED IN KNOXVILLE, TN 

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Patient Engagement Coordinator monitors patient rosters with insurance companies and within the University Family Physicians' clinical practice to engage patients in their healthcare and assist the physicians/practice in meeting quality program metrics. This position facilitates and communicates with the UT Internal Medicine and OBGYN Center to engage patients in their healthcare.

 

DUTIES AND RESPONSIBILITIES:

  1. Schedule patient appointments, discuss health plans, discuss health barriers to the health plan, and transportation needs.
  2. Coordinates with the Quality Director, Quality Manager, and Quality Coordinator to ensure workflows are established in the clinical setting to address the needs of patients.
  3. Communicates directly with TennCare insurance companies to gather patient rosters, ED utilization lists, and inpatient utilization lists for panel management.
  4. Markets and recruits for the Patient Advisory Council; communicates monthly with patients and employees who are members of the Patient Advisory Council.
  5. Directs, UFP team members and Patient Centered Medical Home Team regarding scheduling appointments, linking patient and family/guardian to community-based organizations and services, follow up with/support for patient and family/guardian, communicating with/support for providers, assisting with patient scheduling and transportation needs.
  6. Leads and facilitates the High-Risk Population Management monthly meetings.
  7. Participates and collaborates with providers, clinical staff, operational leadership, and care coordination team members.
  8. Research resources and liaison with University Health Network, University Health Systems, and other community resources to provide physician/ nurse care team resources to address the needs of patients and/or caregivers.
  9. Presents updates during PCMH Power Hour and Practice Management monthly meeting to educate and train physicians and nurses on available resources to provide point-of-care help with patients for non-clinical issues.
  10. Performs other duties as required.

MINIMUM REQUIREMENTS:  

EDUCATION: Bachelor's Degree in a health-related field or a Registered Nurse. (TRANSCRIPT REQUIRED)

EXPERIENCE:  Two (2) years of experience in a healthcare setting; OR a combination of work experience and college coursework to equal six (6) years. 

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent interpersonal, organizational, and analytic skills required to accomplish assessment calls and coordination/prioritization of interventions.
  • Knowledge of HIPAA and ability to maintain and ensure the confidentiality of all Personal Health Information (PHI) collected and disseminated, in accordance with HIPAA requirements.  Ability to maintain strict confidentiality of client, company, and personnel information.
  • Time management and multitasking skills including the ability to keep up with multiple tasks/lists.
  • Ability to train/teach a diverse group of people.
  • Knowledge and ability to utilize Microsoft Office programs (Word, Excel, PowerPoint).
  • Excellent oral and written communication skills.
