Compensation (Based on experience and qualifications): $19.00- $23.00 per hour.
The Behavioral Health Care Coordinator (BHCC) works within the Behavioral Health department, supporting patients referred to and/or participating in the Pediatric and Adolescent Transformative Healthcare (PATH) Collaborative (see attached). This role is responsible for triaging, coordinating, and supporting Esperanza pediatric patients receiving behavioral and/or developmental services through the collaborative. The PATH Collaborative BHCC also supports the child and adolescent psychiatry provider clinic and pediatric patients receiving psychiatry services at Esperanza.
The position is best suited for mission-driven, motivated, and organized individuals with experience and/or interest in working in community care settings and the pediatric mental health field.
Primary Duties and Responsibilities:
- Work as part of a care team to coordinate initial and follow up care for pediatric patients engaged in psychiatry and/or Behavioral Health services as part of the PATH Collaborative.
- Assist the psychiatry provider by recording patient vital signs (weight and blood pressure) prior to their initial and follow-up psychiatry appointments, as necessary.
- Maintain timely and consistent communication with patients and caregivers regarding their Health Care and resource needs, with an emphasis on medication access and adherence.
- Facilitate communication between patients, caregivers, psychiatry provider, Behavioral Health providers, primary care physicians, nurses, medical assistants, and other outside resources related to the care of patients.
- Facilitate timely data collection and sharing using the Clinify platform to evaluate PATH Collaborative outcomes.
- Use assessment results and provider input to refer patients and their families to community resources and/or entitlements (e.g. health insurance services, public assistance, transportation, childcare, employment, nutritional/exercise classes, etc.) and to ensure services are received.
- Work closely with PATH member organizations regarding referrals and connecting referred patients and families to recommended services and resources.
- Help patients to connect to primary care, specialty, and ancillary (e.g lab/diagnostic) services by providing relevant information and making appointments, as needed.
- Encourage patients to become actively engaged in their own health through calm, compassionate communication, and by utilizing motivational interviewing techniques.
- May participate in PATH committees or subcommittees to inform collaborative development and success.
- Engage in community outreach with neighborhood organizations, social service providers, places of worship, and other business partners.
- Meaningfully participate in regular administrative and clinical supervision and team meetings.
- Work as part of a team to address other needs related to Esperanza's mission and vision.
- Day to Day: Huddling (communicating) with care team, reviewing patient charts, identifying gaps in care, identifying areas of need, scheduling appointments, participating in the establishment of a care plan, ensuring fidelity to the care plan.

Requirements
- Bachelor's Degree or equivalent work experience.
- One+ years of experience.
- Ability to speak, read, and comprehend English and Spanish required.
- Previous experience in a community Health Care setting, including work as a medical assistant, case manager, and/or health educator, preferred.
- Previous experience working in a mental health setting preferred.
- Experience working with medically underserved populations, particularly those impacted by social inequities and injustices.
- Strong interpersonal and communication skills; previous customer service experience a plus.
- Strong ability to multi-task and work well in a fast-paced environment.
- Ability to take initiative and work autonomously; knows how to ask for help and support when needed.
- Ability to organize and prioritize appropriately. Attention to detail is necessary.
- Ability to adapt to change and respond to unforeseen situations or circumstances.
- Computer skills required. Prior experience using Excel and electronic health records systems is preferred.
- Function as a team member to assure patients obtain access to quality care.
Pediatric and Adolescent Transformative Healthcare (PATH)
The Pediatric and Adolescent Transformative Healthcare (PATH) Collaborative will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago. The purpose is threefold:
- To expand access to developmental health providers to conduct assessments and provide services including occupational, speech, and physical therapy.
- To expand access to pediatric Behavioral Health providers to address the growing mental, emotional, and Behavioral Health needs of children and adolescents.
- To address the social and structural determinants of health through care coordination, case management, peer support services, home visiting, policy and advocacy, localized hiring, workforce development, data sharing, and collaborative learning.
PATH is led by Saint Anthony Hospital, a community-based safety net hospital, and includes federally qualified health centers (FQHCs), an inpatient pediatric Behavioral Health hospital, community-based organizations, a Business Enterprise Program (BEP) population health tech company, a BEP fiscal intermediary, and an evaluator specializing in assessment through a racial equity lens. PATH will utilize an interdisciplinary, shared model of care to address inequities surrounding pediatric behavioral and developmental health and the social determinants of health. Focusing on the integration of primary and Behavioral Health Care services, improved transitions of care from inpatient to community-based settings, and expanded access to prevention-based services, PATH will deliver programming through four streams of services:
- Behavioral Health - Psychiatric diagnostic evaluations; Medication management; Individual, family and group psychotherapy; Social emotional learning groups; Psychiatric eConsult's to FQHCS
- Developmental Health - Developmental screenings; Home Visiting (Parents as Teachers); Physical, occupational, and speech therapy; Parent socialization groups.
- SDOH Services - Care Coordination (School, transitions of care, clinical); Peer Support; LGBTQ+ care management; Mental health helpline; Localized hiring.
- Integrative Services - Data sharing platform; Evaluation through a racial equity lens; Learning collaboratives; Education and advocacy.
