Crestwood Terrace is an Intermediate Care Facility (ICF) with 126 beds and is providing patient care to the mentally ill/behavior health population (ages 21 and up). Located in a quiet neighborhood, we are one of 3 medical centers on our street. What makes us unique:
The National Association of Rehab Providers & Agencies (NARA) awarded Transitional Care Management their "Impact Award" for 2023 for our Life Skills Programming. This center is using that very same "Life Skills Program" and changing lives every day. Ask us about it during your interview - we'd be delighted to share our Social Services success stories. Job Description
Assistant Director of Nursing (DON)
What you will do in this role: The Assistant Director of Nursing (ADON) will operate as a significant support to the DON. You will be training and orienting new staff members, supervising and leading others as well as being involved with specialty program development. This leadership position will be integral to the success and retention of our team.
Every day is a little different than the next.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.

Qualifications
Assistant Director of Nursing (DON)
What we need from you:
- Registered Nurse (RN)
- 1-3+ years of experience as an ADON, Charge Nurse or Nursing Supervisor in an ICF or SNF setting is ideal
- Experience leading: restorative, wound care, ICP, falls prevention and pain management or other specialty programming is helpful
- Strong CNA leadership experience
- PCC documentation experience
