SSM Health Dean Medical Group has a rewarding career opportunity for a full-time (1.0 FTE) Palliative Care Nurse Practitioner to join our specialty Palliative Care program across the continuum in Madison, WI. The Palliative Care Nurse Practitioner works in collaboration with the cardiology and pulmonary providers and other interdisciplinary team members. This provider is accountable for providing specialty Palliative Care to cardiology and pulmonary patients and their families in the clinic and hospital. The ideal candidate will be patient-centered, service-oriented and will demonstrate the competencies of a Palliative Care provider.
Highlights:

  • Currently no nightly or weekend call requirements
  • 1.0 FTE (5 days)

Minimum Qualifications:
  • Advanced Certified Hospice Palliative Nurse (ACHPN), if not already obtained, within two years of hire
  • Minimum of two years of clinical experience in Palliative Care setting as either an RN or NP
  • Active unrestricted WI RN and NP license
  • Active unrestricted WI DEA
  • Team player, strong collaborator, strong communication skills

Job Summary:
In collaboration with physician(s), demonstrates and provides professional nursing care to specialty patients requiring an advanced level of knowledge and management of their nursing care.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
  • Initiates and provides medical interventions in response to acute changes in patient conditions.
  • Provides educational services to nursing and ancillary departments and serves as an educational consultant on clinical practice issues.
  • Applies evidence based practices to achieve multidisciplinary collaboration to facilitate continuity of care.
  • Contributes to the satisfaction of the medical staff by collaborating with physicians to deliver best care.
  • Recognizes and utilizes opportunities to act as a change agent by introducing nursing practice innovations.
  • Evaluates nursing practices associated with specialty area, defines needs/problems with patient care and makes constructive suggestions that creates a positive patient outcome.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

EDUCATION
  • Completion of a formal post-basic education program from or formally affiliated with an accredited college, university or hospital (Before July 1, 1998) or a graduate degree from an accredited college or university (After July 1, 1998) with a concentration in the appropriate advanced practice nursing clinical specialty area.
  • Board certification by a national recognized certifying board in the appropriate population focus
  • Current or compact RN license in appropriate state
  • Appropriate APRN license or document of recognition in appropriate state

EXPERIENCE
  • One year experience

PHYSICAL REQUIREMENTS
  • Constant use of speech to share information through oral communication.
  • Constant standing and walking.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, reaching and keyboard use/data entry.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of smell to detect/recognize odors.
  • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Occasional lifting/moving of patients.
  • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
  • Occasional driving.
  • Rare crawling and running.
