We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services.
The Triage Nurse is primarily responsible for managing and triaging urgent and emergent patient calls by applying nursing knowledge and standards to determine next steps to address caller's needs and concerns to include transfer of call to PAC agent to schedule future or immediate appointment, connecting with emergency services, personnel or 911 services, etc. The position is responsible for establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements, as well as triaging calls according to guidelines and in a manner that promotes and assists with restoration of patients' health and scheduling, coordination of care, and follow-up as needed. The Triage nurse will assist with the management of provider-to-provider calls, skilled visiting nurse-to-primary nurse calls, and pharmacy liaison calls. The triage nurse manages calls for adult, pediatric, and prenatal clients and completes required documentation. Contributes to the team effort by supporting all team members and maintaining an open and positive attitude. Welcomes new employees and students to the team by assisting with their orientation to the team and mentoring them, as requested. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. Every employee shares the responsibility for promptly bringing to the County's attention conduct that interferes with providing a work environment free of illegal discrimination and harassment.
As a newly hired Community Health Nurse you will receive a signing bonus of $2,500 prorated based on FTE (0.8 FTE and higher will not be prorated). This bonus will be paid in one lump sum on the next regularly scheduled pay date after you start employment with Multnomah County. Furthermore, there will be a retention bonus of $2,000 prorated based on FTE (0.8 FTE and higher will not be prorated) after one year of continuous regular employment. This signing bonus is taxable, and all regular payroll taxes will be withheld.
As the Community Health Nurse, you will provide:
Care Coordination:
  • Triage patients with medical, mental, behavioral, and dental health emergencies and/or present urgency. Apply nursing knowledge and standards to determine next steps to address caller's needs and concerns to include transfer of call to PAC agent to schedule future or immediate appointment, connecting with emergency services, personnel or 911 services, etc.
  • Follow suggested script for asking questions, gathering information, listening for client concerns and ability to communicate need to evaluate urgency and next steps for possible treatment plan or referral to internal or external resources and services. Complete screening and comprehensive assessment of on-going health issues (e.g. depression, diabetes, asthma, wound and pain management, and other chronic diseases). Liaise with outside specialists, hospitalists, skilled visiting nurses, and school nurses, and internally with medical, behavioral health, and dental providers to gather information, round table client concerns, and develop recommendations for care plan, next steps for client care and needs.
  • Uses professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information/education, referral and evaluation. Provide clinically based advocacy, counseling, teaching, problem solving, and selection and utilization of resources, while respecting client preferences, social, cultural, spiritual and health beliefs. Demonstrate professionalism by appropriate attire, attendance, attitude, and behavior within the clinical setting.

Team Collaboration:
  • Maintains clinical assessment skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine). Reviews self and team key performance indicators and contribute to team goal setting.
  • Completes competencies required for the position.
  • Attend team meetings and periodic department meetings. Assists in the welcoming, training, and mentoring of new employees and transfers to understand and be aware of words, phrases or manner of communicating that may indicate an urgent or safety situation that may require clinical and nursing based evaluation.
  • Serves as a conduit for patients, PAC agents, and clinic teams in the assessment of care needs and screening of phone calls and triage messages and works to address questions and concerns relative to scope of practice.

Quality Assurance /Improvements:
  • Participates and supports the quality improvement process through team, role, and designated specialty work groups. This position will incorporate safe work practices with each task performed and actively participate in initiatives designed to promote a safe work environment for team members who may encounter stressful or unsteady callers. Shared responsibility with the clinic leadership team to apply continuous quality improvement using quality based tools and approaches to systematically review program clinical operations and implement improvements. Apply nursing knowledge and experience to improve the caller's experience, appropriate gathering and documentation of medical information, recommendations to match scope of practice and licensure levels, etc.

Customer Service:
  • Shared responsibility with the clinic leadership team for creating an environment of accountability of staff towards customer service and managing patient complaints as needed. Foster a culture of transparency around client sanctification surveys and responsiveness, factoring in employee feedback, review and participation for full team involvement. Partners with clinical leadership teams to develop and implement shared goals to ensure service excellence for internal customers, external customers, and community partners. Position may participate on internal or external committees or community based teams to partner for services and joint ventures to support community wellness and coordinated care.

Minimum Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
  • Possession of an appropriate and valid license to practice as a Registered Nurse issued by the State of Oregon.
  • BLS/CPR certification required
  • Ability to work independently
  • Ability to work in an interdisciplinary setting fostering a safe, welcoming and inclusive environment
  • Experience providing services to diverse and vulnerable communities

Teleworking (if applicable): At this time, this position is designated to "hybrid telework," meaning you will be working remotely (from home) most of the time. As with all telework positions, employees may be required to come into the office on occasion or visit a location in the community. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other states.
Salary/Pay:Please be advised that the pay range listed for this position is intended to provide general guidance on the earning potential for the role. However, actual compensation will be determined in accordance with the Oregon Equal Pay Law and will take into account factors such as the candidate's relevant experience and education. Candidates should expect that initial offers will be made within the listed pay range and may not be at or near the top of the range.
