The Community Health Nurse (CHN) provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information / education, referral and evaluation. This position is with the Healthy Birth Initiative (HBI) program and is a home visiting program working with the entire family to decrease health disparities in the African American community and requires lived experience working with the African American culture. This position may telework as needed to ensure no lapse in client services.
As the Community Health Nurse, you will:
  • Proactively assess, plan, implement and evaluate healthcare services that improves and promote healthy outcomes
  • Provide regular screening, comprehensive assessment and interventions for on-going health related to pregnancy, postpartum care, infant child growth and development
  • Provide education through classes and individual care.
  • Assure completion of referrals and coordinate care with other healthcare providers
  • Assess clients needs for in-person and telephone response for urgent and non-urgent medical information
  • Maintain clinical skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine)
  • Seek expert advice on issues for clients and provide input to providers and nurses on client response
  • Complete documentation and charting as required by policy
  • Safely assess patients referred to the program, including but not limited to full assessments, psychosocial assessments, focused assessments, and options counseling and the intervention required for each
  • Mentor, coach and offer clinical expertise to team members
  • Delegate the care of individuals and groups of clients to members of the team based on the scope of practice and clinical skills needed by the clients
  • Assure appropriate client follow-up and foster community partnerships to best meet the needs of the client
  • Participate and support the quality improvement process through team, role, and designated specialty workgroups
  • Incorporate safe work practices with each task performed and actively participate in initiatives designed to promote a safe work environment

Minimum Qualifications/Transferable Skills*:
  • Equivalent to a bachelor's degree
  • one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position
  • License to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021.
  • This position requires advanced knowledge, skill and ability and has experience working with the Black/African American community. Demonstrates an understanding of the traditions, values and customs of this community. Understands cultural/racial/ethnic factors, including systemic advantage or disadvantage, generational trauma, and any factors related to non-dominant culture affecting Community Health.
  • A valid BLS/ CPR card or the ability to obtain certification within 60 days of hire is required
  • Valid state issued driver's license

  • Deep understanding of disparities that inform equity driven approaches to the work

Hybrid Telework: This position is designated as "hybrid telework," meaning you will be working both remotely (from home) and in person at an onsite location. The designation of Hybrid Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.. The onsite work location for this position is Walnut Park Complex, 5329 NE MLK Jr Blvd, Portland, OR 97211.
