TITLE: Outreach Peer Support Specialist

LOCATION: Yuma, AZ

SUMMARY: Full-time position that works closely with the Sgt. Fox interdisciplinary team with a special emphasis on preventing Veteran suicides. Will perform a variety of supportive tasks and assisting the Veteran achieve theirs Service Plan Goals.

QUALIFICATIONS:

• Preferred Honorably Discharged Veteran.
• Minimum High School Diploma and 1 year experience as an Outreach Specialist.
• Valid state driver’s license.
• Knowledge of community resources.
• Ability to speak with individuals in an open and welcoming manner.
• Moderate writing skills.
• Preferred Peer Support Certification.
• Able to maintain boundaries.
• Pass background check and finger prints.
• Maintain regular attendance.
• Able to perform work activities in a high pressure and time sensitive environment.
• Reliable vehicle and insurance.

RESPONSIBILITIES:

• Perform outreach in Yuma County.
• Work with Case Managers and Veterans in achieving goals.
• Documents Veteran contacts for accurate reporting for data entry and evaluation activities.
• Flexible and accessible to Veterans, and performs wellness checks.
• Performs partial intakes.
• Maintain constant contact with Veterans and if needed family members.
• Attend staffings.
• Assists with establishing working/referral networks.
• Other duties as requested.

• Bilingual proficiency (English/Spanish).
• Experience working with Veterans.
• Able to share life experiences that resulted in positive outcomes and wellness.

• Able to travel in Yuma County.

Job Type: Full-time

Pay: $19.91 - $21.95 per hour

Expected hours: 40 per week
