Outreach Coordinator Full-Time

The Outreach Coordinator is responsible for the client’s plan of car and communicating with the HR manager as needed to assure safe and effective coverage of client needs. Signing up clients for homecare services. Building relationships in the community and planning events.

Qualifications:

• Candidate must have a minimum of 2 years’ experience in a related field or at least an associate degree (preferred).
• Candidate must have strong communication skills.
• Must be able to drive for long periods of time, up to 1hr. and 30-minute drives.
• Candidate must be able to quickly resolve people’s problems.
• Candidate must be familiar with database systems.
• Candidates must be able to work M-F; 8-4 pm and on-call.

Essential Functions and Basic Duties:

• Sign up clients.
• Do supervisory visit every quarter.
• Weekly calls to check in on clients (not to replace Supervisory visits)
• Follow-up with the client twice a week until the client is staffed and once a week after client is to confirm satisfaction.
• Document
• Communicate with case managers regarding client hours, staffing, concerns, and anything else related to the client.
• Manage clients care and notify clients of changes.
• Advocate for the client.
• Investigate clients’ issues.
• Notifies Human Resource Department of staff needs.
• Visiting senior homes and rehab centers
• Plan events with senior homes and rehab centers.
• Visit community organizations and build relationships with them.
• Following up with referrals.
• Updating spreadsheet timely.
• Performs other related duties and responsibilities as deemed necessary.

Job Type: Full-time

Pay: $19.00 - $21.00 per hour
