Position Purpose: To implement a robust outcomes system and aggregation of outcomes data for all Rosecrance programs. To implement routine performance improvement functions in accordance with written agency policies and procedures and accreditation standards.

Qualifications/Basic Job Requirements:

  • HS Diploma or GED
  • Minimum of three years working with customers or clients, preferably in a healthcare setting
  • Must by proficient with computers and database management
  • Must have experience and mastery of Microsoft Word and Excel.
  • Preferred two years’ experience in database management and data analysis
  • Preferred three years Performance Improvement experience
  • Must have knowledge of The Joint Commission, DASA, DMH and/or other regulatory standards
  • Training or experience in statistical information gathering and root cause analysis
  • Excellent verbal and written communication skills

Essential Responsibilities:

  1. Work hours prescribed by the Performance Improvement Director and any additional hours deemed necessary for the responsibilities assigned
  2. Establish, maintain, and manage a web-based portal for outcome tools and survey measurements.
  3. Assist clients with access client portal, including technical support and troubleshooting any problems.
  4. Assist staff with access individual client reports from the client portal, including technical support and troubleshooting any problems.
  5. Administer outcome tools with individual and groups of clients as needed either via in person or by telephonic interviews.
  6. Perform data calculations and routine analytical functions relative to performance improvement activities.
  7. Identify, evaluate, and correct errors caused by inappropriate decision making, human behavior, leadership practices or organizational weaknesses that impact safety and/or operational performance.
  8. Results of weaknesses that impact the organization are to be incorporated into training methods and procedures to support an organizational culture of continuous improvement.
  9. Train and mentor personnel at various levels across the organization in the handling and use of client outcomes data into clinical practices.
  10. Collect and analyze company wide client outcomes and staff performance data in order to provide conclusions and recommendations to Senior Management to support improved processes
  11. Assist organization in meeting requirements for Joint Commission and state licensing surveys
  12. Maintain up to date knowledge of regulations affecting the organization and act as a resource to staff
  13. Assist Performance Improvement Coordinator in reviewing and revisions of the Policy and Procedure Manual, Safety Policies, Risk Management Policies and Infection Control Guidelines.
  14. Perform audits as requested by the Director of Performance Improvement
  15. Assist in Quality Assurance and Focus Medical Review under the direction of the Director of Performance Improvement
  16. Assist Director of Performance Improvement on any specific PI/Quality related tasks, research, and help to develop educational material for identified service needs
  17. Assume other related responsibilities as assigned by management.

Job Type: Full-time

Pay:

  • Starting pay at HS/GED - $18.32/hr
  • Pay rate increase based on education and experience

Schedule: 8 hour shifts

  • Shift: 8:30am - 5:00pm
  • Monday through Friday

Work Location: Rosecrance Rockford East – Rockford, IL
