JOB SUMMARY: Lifeline Connections is looking for a Peer to join the Opioid Treatment Network team locating in PeaceHealth SW Medical Center. The Peer with the OTN will work with an interdisciplinary team including doctors, social workers, nurses, and peer support specialists, all with the common goal of assisting patients suffering from a substance use disorder. Job is a unique opportunity for someone passionate about working with those struggling with addiction as the job is stationed in the hospital.
MAJOR DUTIES/RESPONSIBILITIES:
This is a hospital-based position and will provide care coordination and clinical services to hospital patients. The OTN peer will work closely with the on-site supervisor to make sure patients are assessed for MAT services. In fulfilling these duties, the incumbent performs the following duties independently:
- OTN Peer will educate community partners/hospital patients about MAT services.
- Will screen and assess individual patients to determine the need for MAT services.
- OTN Peer will continue to meet with MAT patients throughout their hospitalizations to support patients engaged in MAT services.
- Will facilitate successful and appropriate follow-ups for hospital MAT referrals to continue outpatient MAT services at community clinics.
- OTN Peer will document patient progress and recommendations in the electronic health care system. All in-person and telephonic encounters will also be documented.
- Gather required date and report date to necessary staff.
- Other duties as assigned.
POSITION REQUIREMENTS:
- Minimum of an associates in a social service field with 1 year of experience required; 3 years’ experience in a social service field in lieu of degree.
- Certified peer.
- Above average Microsoft Office skill set.
- Experience working within health systems.
- Experience with underserved, recovery and mental health populations.
- Familiarly in Motivational Interviewing.
- Strong communications skills.
- Ability to professionally communicate verbally and in writing.
