Job Description Summary:
This position performs patient care by assessing patients physical and functional abilities in the HomeCare Department. Based on problems determined, the Occupational Therapist establishes and carries out plan of care to include: patients/family instruction, d/c planning, documentation in medical record, communication with team, and reassessment. He/She helps patients achieve maximum level of functioning. This position acts as clinical instructor for students, and participates in departmental functions, i.e., process improvement.
Minimum Qualifications:
Bachelor's Degree: Occupational Therapy (Required)DL - Driver's License - Department of Motor Vehicles, LIC - Licensed in Field - State of Ohio
State Driver's License. Licensed in the State of Ohio.
