Occupational Therapist- SCS Home Health

Schedule: Monday- Friday, 8:00am - 5:00pm

Service Area: East King County

The Home-Based Acute Care Occupational Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Occupational Therapist may also be assigned to orient staff and students. The Occupational Therapist works with the patient and family/caregivers in the home setting without direct supervision.

Required Qualifications:

  • Bachelor's Degree In Occupational Therapy from an institution approved by the state board in collaboration with the American Occupational Therapy Association.
  • Upon hire: Washington Occupational Therapist License
  • Within 30 days of hire: National Provider BLS - American Heart Association
  • Within 30 days of hire: National Provider Identifier (NPI) number
  • 1 year Therapy experience with adult geriatric and/or individuals with physical disabilities

Preferred Qualifications:

  • 1 year Therapy experience in home care or in related rehabilitation setting.
  • Experience with the geriatric population.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
