Essential Responsibilities

• Notre Dame Home Health is seeking a part time Occupational Therapist for the Metro New Orleans area and the west bank of Orleans and Jefferson Parishes. 
• OT will make home visits in the field to assigned patients to deliver OT services as ordered by a physician, NP or PA. 
• OT is responsible for electronic charting on Wellsky software for evaluations, follow up visits, verbal orders, re-certifications, re-evaluations, and discharge summaries. 
• OT is responsible for carrying out the assigned care plan and visit frequency ordered by the licensed provider and to monitor patient's responses to treatment and make changes as needed. 
• OT is responsible for compliance with state and federal regulations and is responsible to the agency administrator. 

Qualifications

• Applicants must possess a current Louisiana OT license and must have one year of clinical experience as a licensed OT. 
• Applicants must possess good communication and computer skills and be able to manage all documentation in the electronic medical record.
• Must possess a current drivers license in good standing, a vehicle in good working condition, and current auto insurance are all required. 
• Applicants must have a current CPR card and have current covid vaccination unless a valid Medical exemption or religious exemption is provided.
