OASIS RN Responsibilities:
As a Home Health Admission Nurse, your duties will include:
- Performing Admission / "Start of Care" OASIS Visits throughout the Metro area.
- Evaluating and assessing clients for home health services and creating customized plans of care.
- Recommending additional services from other disciplines as needed and collaborating with the healthcare team.
OASIS RN Qualifications:
To succeed in this role, you'll need:
- A valid Registered Nurse license in the state of Minnesota.
- A reliable vehicle, valid Driver's License, and current insurance.
- Prior experience in Home Health Nursing and proficiency in Medicare & OASIS documentation.
- Strong autonomy, organization, and communication skills.
