Responsibilities:
- Nurse care management: assess, plan, implement, coordinate, monitor, and evaluate the plan of care with the attending physicians.
- Provide well-documented, accurate and sensitive in-person advice, health education and self-care instruction.
- Support patients during times of crisis or transition to prevent avoidable hospital readmissions.
- Reach out to patients identified as high-risk.
- Speaks with patients over the telephone to assess their medical needs, directs them to ER/Urgent care or the physician's office if during office hours.
Qualifications:
- Valid LVN license.
- Ability to thrive in a fast-paced environment and adapt to change.
- Ability to work both independently and collaboratively within a team.
- Demonstrated clinical assessment skills.
