Purpose of Position
The job duties of the Nutrition & Transportation Programs Supervisor include but are not limited to develop, implement and coordinate Nutrition and Transportation Programs policies and procedures; to perform programming and administrative functions to assure programs meet State and Federal regulations to ensure continued program funding, to supervise, train and direct Program staff to follow food and sanitation requirements in the field; and to provide responsive, courteous and efficient service to County residents and the general public.

Essential Duties and Responsibilities
DUTIES AND RESPONSIBILITIES
The duties described below are indicative of what the Nutrition & Transportation Programs Supervisor might be asked to perform. This job description is to incorporate any county ordinances created for the position of Nutrition & Transportation Programs Supervisor. This is not an exhaustive list of job responsibilities and therefore other duties may be assigned:
Administrative Functions:
  • Determine eligibility for nutrition and Transportation Programs, enroll participants and record in consumer databases as appropriate for program. Conduct or arrange initial and annual home visits for home delivered participants, and record assessment information into the SAMS/Wellsky program database.
  • Work with dietitian to provide nutrition education to home delivered and congregate program participants.
  • Supervise, educate and direct site staff, volunteer drivers and family members to ensure meal recipient's safety.
  • Ensure monthly donation letters are sent to home delivered meal and Senior Dining recipients or designated persons.
  • Develop and recommend to the ADRC Manager, Human Service Director and ADRC Board all Nutrition and Transportation Programs policies and procedures.
  • Implement federal, state and local policies and procedures for the nutrition and Transportation Programs.
  • Oversee day-to-day nutrition and transportation operations, including providing guidance and leadership to staff; training, coordinating work assignments, scheduling, monitoring performance, completing performance reviews, approving time off, and approving payroll.
  • Make recommendations to and assist ADRC Manager with preparing nutrition and Transportation Programs annual budgets based on anticipated changes in state and federal policies, procedures and priorities.
  • Identify agencies/vendors that can best meet customer needs and coordinate with ADRC Manager and human services staff to facilitate contracting process.
  • Provide ongoing quality assurance of programs and make recommendations to ADRC Manager with regard to major program changes.
  • Coordinate with ADRC Manager and Human Services Fiscal/Contracts Division to establish yearly provider contract rates and develop outcomes to ensure providers focus on delivering results that meet nutrition and transportation consumer needs.
  • Work with Family Care MCOs and IRIS ICAs to facilitate the provision of meals for customers.
  • Maintain the following for the Nutrition Program: staff records verifying training and required certification, monthly roster sheets for monthly recording proposes, weekly route sheets for daily delivery purposes, and volunteer driver documents.
  • Coordinate with the Division Manager the implementation of a Nutrition Program Advisory Committee. Ensure that the committee is actively engaged, allowed to provide operational and policy recommendations to both the division and the ADRC Board.
  • Attend required federal, state, and local trainings and implement best practices and evidence-based programming to meet consumer needs and ensure resources are administered efficiently.
  • Develop and submit the 85.21 annual grant to ensure continued funding of services.
  • Coordinate all transportation services under 85.21, which includes ensuring all reports are submitted to the State Department of Transportation on a timely basis, and program participants are properly enrolled and recorded in the consumer database.
  • Review and approve all Vehicle Modification grant applications.
  • Provide monthly articles for newsletter, Bridging Chippewa County.
  • Identify opportunities for increasing program revenue through partnerships, grants, etc.
  • Recruit and train Meals on Wheels volunteers.
  • Use discretion with customers; observe rules of confidentiality and HIPAA compliance; maintain non-judgmental attitude.
  • Participate in ADRC strategic plan and engagement activities.

Customers:
  • All prospective, current or past DHS consumers.

Team Members:
  • Any professional or paraprofessional we work with in the serving of consumers.

Minimum Qualifications
QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required at the time of hire or for the continuation of employment.
EDUCATION AND/OR EXPERIENCE:
  • Bachelors' degree in dietetics, health promotion, business administration or related field with two (2) years recent related work experience required; -OR-
  • Associate degree in business administration, dietetics or related field with six (6) years of recent related work experience required; -OR-
  • High School Diploma with ten (10) years direct experience with coordinating Older Americans Act programs required.
  • Supervisory experience preferred.
  • Must obtain the ServSafe Food Manager Certification within 6 months of hire.
  • Knowledge of Wisconsin Food Code preferred.
  • A valid driver's license required.

Chippewa County offers a comprehensive benefit package including:
Health Insurance - The County's Health Plan is administered by AmeriBen w/ a $2,000 deductible for an individual and $4,000 deductible for a family. There is NO employee monthly premium for single and family coverage. Health Reimbursement Account (HRA) contributions are made quarterly by the County equal to $600 per person per year on the plan with a maximum of $2,400 annually. AmeriBen's provider network is Anthem Blue Preferred and includes Mayo Clinic Health System, Marshfield Clinic, Prevea, Oakleaf, Sacred Heart, St. Joseph's, and more!
Wisconsin Retirement System: Employee is required to contribute 6.50% with a County match of 6.50% in 2022. This percentage may change each year based on state requirements.
Other insurance plans offered: Dental, vision, life, accident, critical illness, short-term and long-term disability and flexible spending accounts.
PTO accrual equal to 7.09 hours of PTO per pay period (26 pay periods in a year).
9 paid holidays per year.
